چکیده
مقدمه: درمان بهینه شکستگی‌های پیچیده و نقایص بزرگ استخوانی موضوع مهم حل‌نشده‌ای در ارتوپدی و تخصص‌های مرتبط با آن است. راهکار درمانی استاندارد برای تقویت بازسازی استخوان برای نقیصه‌های کوچک‌تر از 5 سانتی‌متر، پیوند مستقیم استخوان است. هدف از این مطالعه بررسی رادیولوژیک و هیستوپاتولوژیک تأثیر استفاده پودر کیتوزان بر ترمیم استخوان در مدل موش صحرائی است تا با مطالعه روند بهبود و جوش خوردن استخوان، مزایا و معایب این روش معرفی گردد و بتوان در آینده داربست‌هایی با استحکام کافی برای درمان شکستگی‌های این‌چنینی معرفی کرد.
مواد و روش ها: تعداد 15 موش صحرائی نر در یک محدوده سنی و وزنی انتخاب شدند و به 3 گروه پنج تایی تقسیم شدند. در تمام موش‌ها 3 میلی متر از متافیز استخوان رادیوس برداشته شد. در گروه اول، محل نقیصه، صرفا بخیه زده ‌شد. در گروه دوم محل نقیصه با اتوگرافت استخوانی جایگزین ‌شد و پوست بخیه زده ‌شد. محل نقیصه در گروه سوم با پودرکیتوزان پر ‌شد. در ادامه در هفته چهارم و هشتم پس از عمل، از محل جراحی عکس رادیوگرافی تهیه ‌شد و بعد از نگهداری از موش‌ها در پایان هفته ی هشتم (روز 56)، به روش انسانی معدوم سازی ‌شدند و برای بررسی های هیستوپاتولوژیکی، نمونه برداری صورت ‌گرفت.
نتایج و بحث: یافته‌های رادیوگرافی و هیستوپاتولوژیکی بهبود قابل‌توجهی در کیفیت و کمیت استخوان تازه بازسازی‌شده در گروه‌های تحت درمان با کیتوزان را در مقایسه با گروه‌ درمان‌نشده نشان دادند.
نتیجه گیری: نتایج مطالعه حاضر نشان می‌دهد که اثر بازسازی استخوان ناشی از تحریک مکانیکی بافت استخوانی توسط کیتوزان افزایش می‌یابد.

Abstract

Introduction: Bone fracture is one of the most common bone injuries in the world. It is, therefore, not surprising that bone healing is a critical orthopedic problem. Currently, bone grafts (autografts, allografts and xenografts) have been used as bone replacement strategies. The purpose of this study was to evaluate the efficacy of application of chitosan (CHS) as an osteo-inducer in the process of bone healing.
Materials and Methods: Fifteen male rats, in the same age and weight range, were divided into three groups of 5. 3 mm of metaphysis of the radius bone of each animal was removed. The defect was left empty in the first , filled with autograft in the second ,and filled with CHS in the third group. Radiographs of the defects were obtained on 28th and 56th day post-operation. The animals were euthanized according to human ethics and the defects were investigated histologically on the 56th post-operation day.
Results and Discussion: According to radiographic and histopathologic results, CHS indicated a improved effect ,and improved bone healing process in comparison with the other two treatment modalities.
Conclusion: It can be concluded that use of chitosan improves the potential for bone formation and bone healing criteria. These composite materials, however, deserve more studies.
