بررسی تأثیر پوزیشن لگن بر Pelvic incidence

چکیده
چکیده:
مقدمه: با توجه به اهمیت ثابت بودن زاویه pelvic incidence جهت برسی راستای ستون مهره‌ها و از طرفی اهمیت زاویه مناسب جهت فیکسیشن ستون مهره‌ها در اعمال جراحی ستون مهره‌ها، هدف از انجام این مطالعه بررسی این زاویه و مقایسه آن در وضعیت‌های Flexion Full و
Full Extension بوده است.
مواد و روش‌ها: در این مطالعه مقطعی، 44 دانشجو که جهت شرکت در مطالعه داوطلب بودند، مورد ارزیابی دینامیک ویو در دو پوزیشن
Full Flexion و Full Extension قرار گرفتند و زاویه Pelvic incidence در دو پوزیشن مورداندازه‌گیری قرار گرفت. از تمام شرکت‌کنندگان که وارد مطالعه شدند دینامیک ویو در دو حالت 1- نشسته و خم شدن به جلو در حد توان (Full Flexion) و 2- حالت دراز کشیده و سپس پاها آویزان از تخت در حد توان فرد (Full Extension) گرفته شد. سپس با کمک نرم‌افزار PACS زاویه Pelvic incidence اندازه‌گیری شد.
نتایج و بحث: میانگین زاویه PI در پوزیشن‌های Full Flexion وFull Extension ، برابر نیستند و زاویه موردنظر در دو پوزیشن، تغییرات را نشان می‌دهد. تغییرات زاویه PI در پوزیشن‌ها مورد مطالعه بدین‌صورت بود که میانگین آن در پوزیشن Full Flexion بیشتر از پوزیشن
Full Extension بود. میانگین تغییرات زاویه PI بین دو پوزیشن برابر 6/3±9/3 درجه بود. تغییرات به معنی اختلاف زاویه PI بین دو پوزیشن
Full Flexion وFull Extension می‌باشد. میانگین زاویه PI در پوزیشن Full Flexion در زنان 23/11±83/55 درجه به دست آمد که بیشتر از زاویه PI در همان پوزیشن در مردان 62/7±22/53 درجه بود اما به لحاظ آماری اختلاف معناداری دیده نشد (390/0=P). میانگین زاویه PI در پوزیشن Full Extension در زنان 87/9±61/51 درجه محاسبه شد که بیشتر از زاویه PI در همان پوزیشن در مردان 05/7±65/49 درجه بود از نظر آماری، تفاوت معنادار گزارش نشد (608/0=P).
نتیجه‌گیری: نتایج مطالعه نشان داد که پوزیشن لگن بر میزان Pelvic Incidence تأثیر می‌گذارد و اندازه PI در دو پوزیشن‌های مختلف، یکسان نمی‌باشد. همچنین میزان PI در پوزیشن Flextion Full بیشتر از پوزیشن Full Extension بوده است.

Abstract
Introduction: Given the importance of the stability of the pelvic incidence (PI) angle in evaluating spinal alignment and its significance in spinal fixation during surgical procedures, this study aims to investigate the PI angle and compare it in two positions: Full Flexion and Full Extension.
Materials & Methods: This cross-sectional study assessed 44 student volunteers using dynamic radiographic evaluation in two positions: Full Flexion and Full Extension. Dynamic radiographs were taken in two conditions: (1) seated with maximum forward bending (Full Flexion) and (2) lying supine with legs hanging off the edge of the bed to their maximum extent (Full Extension). The PI angle was measured using the PACS software.
Results & Discussion: The mean PI angles in the Full Flexion and Full Extension positions were not equal, demonstrating positional changes. The mean PI angle in Full Flexion was higher than in Full Extension. The mean difference in PI angle between the two positions was 3.6±3.9 degrees. In females, the mean PI angle in Full Flexion was 55.83±11.23 degrees, which was higher than in males (53.22±7.62 degrees), though this difference was not statistically significant (P = 0.390). In Full Extension, the mean PI angle in females was 51.61±9.87 degrees, which was again higher than in males (49.65±7.05 degrees), but the difference was not statistically significant (P = 0.608).
Conclusion: The study results indicate that pelvic positioning affects pelvic incidence, and PI measurements are not constant across different positions. Furthermore, PI in the Full Flexion position was greater than in the Full Extension position.
