چکیده
مقدمه: هدف از انجام این مطالعه یافتن ارتباط آماری میان یافته‌های بالینی و عملکردی بر اساس معیار نمرهدهی Lysholm در میزان موفقیت جراحی و رضایت بیماران از بازسازی آرتروسکوپیک رباط صلیبی قدامی، ACL (anterior cruciate ligament) بود.
روش کار: مطالعه به صورت آینده‌نگر و مقطعی (cross-sectional) انجام شد. 38 بیمار (42 زانو) در ردة سنی 17 تا 45 سال، که طی سال‌های 1397 و 1398 به دلیل پارگی ACL تحت جراحی آرتروسکوپی بازسازی در بیمارستان امام خمینی (ره) اهواز قرار گرفته بودند، وارد این مطالعه شدند. 6 و 12 ماه پس از جراحی، تست‌های کلینیکی شامل «تست کشویی قدامی» (anterior drawer) و «تست لاچمن» (Lachman test) و پرسشنامة استاندارد Lysholm برای بیماران انجام و تکمیل شد و میزان همخوانی نتایج تست‌ها با نتایج حاصل از پرسشنامة Lysholm و در نهایت با MRI بررسی شد.
نتایج: در تشخیص پارگیACL ، 6 ماه پس از جراحی، بالاترین «حساسیت» مربوط به تست کشویی قدامی و معیار Lysholm، بالاترین «اختصاصیت» مربوط به تست لاچمن، بالاترین «ارزش اخباری مثبت» مربوط به تست لاچمن؛ و «بالاترین ارزش اخباری منفی» مربوط به تست کشویی قدامی و معیار Lysholm بود. 12 ماه پس از جراحی، «بالاترین حساسیت» مربوط به معیار Lysholm، «بالاترین اختصاصیت» مربوط به تست لاچمن، «بالاترین ارزش اخباری مثبت» مربوط به تست لاچمن و «بالاترین ارزش اخباری منفی» مربوط به معیار Lysholm بود. در 6 ماه اول، در 16/13% از بیماران نقص ترمیم (graft failure) مشاهده شده بود اما میزان فراوانی بیماران قرار گرفته در دسته Fair و Poor با معیار Lysholm، 95/28% بود. در 6 ماه دوم، در 14/7% از بیماران نقص ترمیم مشاهده شده بود اما میزان فراوانی بیماران قرار گرفته در دسته Fair و Poor معیار Lysholm، 05/19% بود.
نتیجه‌گیری: از آنجایی که در برخی شرایط نظیر زیاد بودن قطر زانو به دلیل چاقی یا وجود تورم، معاینه ممکن است محدود شود و استناد به نتایج تست‌های بالینی ممکن است با خطا مواجه شود، پیشنهاد می‌شود علاوه بر تست‌های مکانیکی نظیر تست لاچمن و تست کشویی قدامی، جهت ارزیابی دقیق‌تر میزان موفقیت جراحی و درمان و نیز کمک به شناسایی دقیق‌تر آسیب‌های محتمل از پرسشنامه‌های استاندارد مانند Lysholm استفاده شود.

Statistical Relationship Between Clinical and Functional Findings Based on the Lysholm Standard Score on the Success Rate of ACL Arthroscopic Reconstruction Surgery

Abstract
Introduction: This study aimed to find a statistical relationship between clinical and functional findings based on the Lysholm scoring scale in the success rate of surgery and patient satisfaction with the arthroscopic reconstruction of the anterior cruciate ligament (ACL).
Method: This prospective and cross-sectional study was performed on 38 patients (42 knees) aged 17 to 45 years who underwent anterior cruciate ligament (ACL) reconstruction during 2018-2019 due to ACL rupture at Imam Khomeini Hospital in Ahvaz.6 and 12 months after surgery, clinical trials of "anterior drawer" and "Lachman test" were performed, and the standard Lysholm questionnaire for patients was completed. Then, the compatibility of the test results with the results of the Lysholm questionnaire and MRI was assessed.
Results: In the diagnosis of ACL rupture, 6 months after surgery, the highest sensitivity was related to the anterior drawer and Lysholm scale tests, and the highest specificity was related to the Lachman test. Also, the highest positive predictive value was related to the Lachman test and the highest negative predictive value was related to the anterior drawer and Lysholm scale tests. 12 months after surgery, the highest sensitivity was related to the Lysholm scale and the highest specificity was related to the Lachman test. The highest positive predictive value was for the Lachman test and the highest negative predictive value was for the Lysholm scale. In the first 6 months, graft failure was observed in 13.6% of patients, but the percentage of patients with“Fair” and “Poor”Lysholm scale was 28.95%. In the second 6 months, graft failure was observed in 7.14% of patients, but the percentage of patients with“Fair” and “Poor”Lysholm scale was 19.05%.
Conclusion: Since in some cases, such as excessive knee diameter due to obesity or swelling, the examination may be limited and citation of clinical test results may be erroneous, it is recommended that in addition to mechanical tests such as the Lachman test and the anterior drawer test, standard questionnaires such as Lysholm be used to more accurately assess the success of surgery and treatment, as well as to help identify more accurate lesions.
