چکیده
پیشزمینه: شکستگیهای ناپایدار مچ پا یکی از متداولترین علل مراجعه به بخش عمومی بیمارستان میباشد و جراحی برای بیحرکتی ناحیه شکسته شده بهعنوان راهکار مناسبی برای بهبود وضعیت بیماران پس از مرخص شدن به اثبات رسیده است. هرکدام از مراحل مدیریت بیمار میتواند بر طول مدت دوران نقاهت و وضعیت بیمار نقش موثری ایفا کند. هدف از این مطالعه صورت گرفته، تعیین نحوه مدیریت بیمار از لحظه پذیرش تا زمان مرخص شدن میباشد و همچنین اثرات محسوس و قابلتوجه تنوع در روشهای مدیریت بیمار نیز مورد بررسی قرار گرفته است.
مواد و روشها: دادههای مورد آنالیز قرارگرفته مربوط به 49 بیمار است که بین آوریل تا دسامبر 2017 به مرکز درمانی نیمهخصوصی HEFT انگلستان مراجعه کردهاند و مشکل شکستگی ناپایدار مچ پا داشتهاند. گفتنی است که تمام مراحل از زمان پذیرش در بخش اورژانس مرکز تا زمان ترخیص مورد بررسی قرار گرفته است. همچنین یادداشتهای بعد از جراحی نیز مرور شدهاند. گرداوری دادهها نیز از طریق منبع TARN انجام شده است.
یافتهها: همانطور که گفته شد، 49 بیمار در مدت ذکر شده به مرکز مراجعه کردند که مشکل شکستگی ناپایدار مچپا داشتهاند. در مدتی که بیماران در بخش اورژانس قرار داشتند برای کمتر از 50% آنها در مدتی کمتر از 30 دقیقه پس از رسیدن به مرکز تشخیص رادیوگرافی انجام شده بود. 25 بیمار نیز پس از حدود 4 ساعت به بخش تخصصی پذیرش شده بودند، در حالی که برخی تا بیش از 9 ساعت مجبور به انتظار شدند. 27 بیمار (بیش از 50%) در حدود 2 روز پس از پذیرش تحت عمل جراحی قرار گرفتهبودند، و 6 نفر هم بیش از 6 روز برای جراحی خود معطل مانده بودند که گروه اول دوره بستری ماندن پس از جراحی کمتری نیز داشتند. برای 47 نفر از بین 49 نفر داروهای ترومبوپروفیلاکسی (ضدلختگی) تجویز شد، به تمامی بیماران توصیه شد که حداقل تا 6 هفته بار سنگینی را روی پای خود وارد نکنند و خوشبختانه هیچگونه عوارض جانبی و مرگ و میر پس از جراحی گزارش نشده است.
نتیجهگیری: درمان سریعتر شکستگی ناپایدار مچپا، دوره بستری در بیمارستان را کوتاهتر میکند.

The Timeliness of Ankle Fracture Management from Presentation to Discharge; Our Experience from a District Eeneral Hospital

Introduction: Unstable ankle fractures are a common presentation and operative intervention has proven to improve patient outcomes. Each stage of patient management can have an impact on patient length of stay and clinical outcome. The aim of the present study is to assess how we manage the patient at each stage from their presentation right through to discharge, and if/how variation in practice differed significantly.
Methods: From April 2017 to December 2017, in the Heart of England NHS Foundation Trust (HEFT), we retrospectively analysed the data of 49 patients with a diagnosis of an unstable ankle fracture. The patient journey was assessed from presentation to the Emergency Department (ED) right through to discharge. Post-operative notes were also reviewed. Data was obtained through our Trauma Audit and Research Network (TARN).
Results: 49 patients had surgical fixation for their ankle fracture. Whilst in the ED, less than 50% of the patients had a radiographic diagnosis of an ankle fracture within 30 minutes of arrival.25 patients were admitted to the ward within 4 hours, with some patients having to wait more than 9 hours.27 patients (more than 50%) had their surgery within 2 days after being admitted, in comparison to 6 patients who had to wait more than 6 days for their operation. The former were found to have shorter hospital length of stay post-operatively.47 out of the 49 patients were given chemical thromboprophylaxis, all patients were advised to not weight bear for a minimum of 6 weeks, and no post-operative morbidity or mortality was encountered.
Conclusion: We found an association between early ankle fracture fixation, and reduced post-operative hospital Length of Stay (LOS).
