چکیده

۳/۳۸پیش‌زمینه: درمان شکستگی جابه‌جا شده گردن فمور در سنین بالا معضل عمده است و نکات مبهمی وجود دارد. هدف این مطالعه مقایسه روش تعویض اولیه مفصل ران با تثبیت داخلی در درمان این شکستگی‌ها می‌باشد.مواد و روش‌ها: در یک مطالعه «کارآزمایی بالینی»، ۸۰ بیمار سنین ۶۰ تا ۷۰ سال که طی ۱۵ ماه به بیمارستان‌های آموزشی‌ـ‌ درمانی شهر اصفهان مراجعه کرده بودند، تحت مطالعه قرار گرفتند. بیماران به صورت تصادفی به دو گروه تقسیم شدند: گروه اول به روش جااندازی و ثابت کردن با پیچ، و گروه دوم با تعویض مفصل ران درمان شدند. شدت درد، کارآیی مفصل ران، کیفیت زندگی و عوارض بیماران در مدت دو سال پس از عمل جراحی، با استفاده از مقیاس دیداری درد، سیستم امتیازبندی «هاریس» و معیار SF36 تعیین و مقایسه گردید.یافته‌ها: شدت درد و میزان عوارض و نیاز به عمل جراحی مجدد در بیمارانی که به روش تعویض مفصل درمان شدند نسبت به گروه دیگر به‌طور قابل ملاحظه‌ای کمتر بود. در گروه اول نمره درد ۴/۵ (از ۱۰)، ۶۸=SF36، امتیاز «هاریس» ۶۱؛ و در گروه دوم نمره درد ۳/۳۸، ۹۲=SF36، امتیاز «هاریس» ۸۶بود. کارآیی مفصل ران و کیفیت زندگی در روش تعویض مفصل نسبت به روش جااندازی و تثبیت بهتر بود. عوارض مختلف جراحی در گروه ۱ در ۱۲بیمار (۴۰%) و در گروه ۲ در ۳ بیمار (۷%) مشاهد شد.نتیجه‌گیری: درمان شکستگی‌های جابه‌جا شده گردن فمور در سنین ۶۰ تا ۷۰ سال با روش تعویض اولیه مفصل هیپ، در دو سال اول با کارآیی بهتر، درد و عوارض کمتری نسبت به تثبیت اولیه با پیچ همراه خواهد بود.

Displaced Femoral Neck Fracture in Elderly Patients: Comparison of Primary Total Hip Replacement with Internal Fixation

Background: Treatment of displaced femoral neck fracture in active, elderly people is a major undertaking with still unsolved issues. The aim of this study is to determine if primary total hip arthroplasty would have as good an outcome as primary internal fixation of the fracture.Methods: In a clinical trial study, 80 cases of displaced femoral neck fracture who referred to the university hospitals of Isfahan, Iran were enrolled into two treatment groups. Group I of 40 patients received screw fixation, and the group II of remaining 40 underwent primary hip arthroplasty surgery. In a 2 year follow-up the surgical outcomes were assessed using VAS pain scale, SF36 general health instrument score, and Harris hip score (HHS).Results: The pain scale (VAS), SF36 and Harris scores showed significantly better results is arthroplasty group compared with fixation group. The values were as follows: Group 1: pain=4.5 (from 10), SF36=68, HHS=61 group II: pain=3.38, SF36=92, HHS=86. Complications were encountered in 16 cases (40%) in group I, and 3 cases (7%) in group II.Conclusions: Displaced femoral neck fractures in 60-70 years old patients treated with primary total hip arthroplasty will have better outcomes in first 2 years, with less complication rates.
