بررسی ارتباط معیارهای رادیوگرافیک پیش از جراحی با میزان آزادسازی مدیال در عمل تعویض مفصل زانو

چکیده
چکیده:
مقدمه: دفورمیتی واروس شایع‌ترین دفورمیتی موجود در بیمارانی است که مورد تعویض مفصل زانو قرار می‌گیرند و اصلاح آن بر آزادسازی بافت نرم مدیال زانو استوار است؛ اما معیار دقیقی برای میزان آن وجود ندارد هدف از مطالعه کنونی بررسی پارامترهای رادیوگرافیک قبل از جراحی است که قادر به پیش‌بینی میزان نیاز به آزادسازی عناصر مدیال می‌باشند.
مواد و روش‌ها: در یک مطالعه‌ی هم‌گروهی گذشته‌نگر کلیه بیمارانی که از در دوره‌ای ۲ ساله در مراکز درمانی دانشگاهی توسط یک جراح مورد جراحی تعویض مفصل زانو اولیه قرار گرفتند به‌صورت سرشماری وارد مطالعه شدند. بیماران بر اساس نوع و میزان آزادسازی مدیال به سه گروه تقسیم شدند. رادیوگرافی استاندارد ایستاده زانو در نماهای رخ و نیم‌رخ و الایمنت قبل و پس از عمل جراحی بررسی شد و معیارهای رادیوگرافیک شامل زاویه مکانیکال فموروتیبیال (mFTA)، زاویه لترال دیستال فمور (LDFA)، زاویه مدیال پروگزیمال تیبیا (MPTA)، زاویه‌ی خط مفصلی (JLCA) و والگوس کات انگل (VCA) قبل و بعد از عمل بین گروه‌ها مقایسه شد.
نتایج و بحث: تعداد ۱۱۵ زانو مرتبط با 103 بیمار موردبررسی قرار گرفتند. بین زوایای لترال دیستال فمورال (LDFA) و مدیال پروگزیمال تیبیا (MPTA) و مکانیکال فمور و تیبیا (mFTA) و والگوس کات انگل (VCA) و میزان نهایی اصلاح واروس (زاویه مکانیکال فمور و تیبیا) با درجه‌ی آزادسازی مدیال ارتباط آماری معنی‌داری وجود داشت (05/0 < P). میزان اصلاح نهایی واروس به‌صورت معنی‌داری پیشگویی‌کننده نیاز به ریلیز وسیع‌تر مدیال می‌باشد (01/0 = P، 49/1 = OR).
نتیجه‌گیری: زوایای LDFA،MPTA ، زاویه‌ی مکانیکال فمور و تیبیا (mFTA) و والگوس کات انگل (VCA) و میزان نهایی اصلاح زاویه مکانیکال فمور و تیبیا با درجه‌ی آزادسازی مدیال ارتباط آماری معنی‌داری وجود دارد و میزان اصلاح نهایی واروس به‌صورت معنی‌داری پیشگویی‌کننده نیاز به ریلیز بیشتر مدیال می‌باشد.

The Association between Preoperative Radiographic Parameters and Medial Soft Tissue Release in Total Knee Arthroplasty

Abstract
Introduction: Varus deformity is the most common deformity in patients undergoing knee replacement. Varus correction is based on the release of the medial soft tissue of the knee. But there is no exact measure of its extent. The present study aimed to investigate radiographic parameters that can predict the need for the release of medial elements.
Materials & Methods: In this retrospective cohort study all patients who underwent primary knee replacement surgery performed in a university hospital by a single surgeon during a 2-years period were included in the study. Preoperative and post-operative knee radiographs were collected. Patients were divided into three groups based on the stage of medial release. The radiographic criteria including Mechanical Femoro-tibial angle (mFTA), Lateral distal femoral angle (LDFA), Proximal medial tibial angle (MPTA), valgus cut angle (VCA) and joint line convergence angle (JLCA) before and after surgery were compared between the groups.
Results & Discussion: A total of 115 knee radiographs, related to 103 patients were examined. There was a significant association between LDFA , MPTA , mFTA , VCA and degree of varus correction with the medial release. The degree of varus correction significantly predicts the need for a high grade of medial release (OR=1.49, P=0.01)
Conclusion: higher LDFA, mFTA, VCA, and the degree of correction of the femoral and tibial mechanical angle and lower MPTA were associated with a higher degree of medial release. And the amount of varus correction significantly predicts the need for more medial release.
