چکیده

پیش‌زمینه: اﻣﺮوزه در ارﺗﻮﭘﺪی داﻣﭙﺰﺷﮑﯽ و اﻧﺴﺎﻧﯽ ﺑﺮای ﺗﺤﺮیﮏ فرایند اﻟﺘﯿﺎم ﺷﮑﺴﺘﮕﯽ‌ﻫﺎ، ﺳﺮﻋﺖ ﺑﺨﺸﯿﺪن ﺑﻪ اﺗﺼﺎل ﻣﻔﺼـﻠﯽ و ﺗﺮﻣﯿﻢ ﻧﻘﯿﺼﻪ‌ﻫﺎی اﺳﺘﺨﻮاﻧﯽ از ﭘﯿﻮﻧﺪ اﺳﺘﺨﻮاﻧﯽ اﺳﺘﻔﺎده ﻣﯽ‌ﺷﻮد. در این مطالعه اثرات استفاده توام نانوهیدروکسی آپاتیت با مغز استخوان در ترمیم شکستگی استخوان خرگوش به شکل رادیولوژیکی و هیستوپاتولوژیکی بررسی می‌شود.
مواد و روش‌ها: این مطالعه تجربی در بهار و تابستان 1398، بر روی 20 خرگوش نر بالغ نژاد نیوزلندی 12 ماهه، به وزن تقریبی 2 کیلوگرم انجام شد. خرگوش‌ها به صورت تصادفی به 4 گروه تقسیم شدند. قطعه استخوانی از استخوان رادیوس برداشته شد در گروه اول (تعداد=5) (گروه کنترل)، نقیصه استخوانی خالی ماند و هیچ ماده‌ای تزریق نشد. در گروه دوم (تعداد=5)، در محل نقیصه نانوهیدروکسی آپاتیت، در گروه سوم (تعداد=5)، مغز استخوان و در گروه چهارم (تعداد=5)، نانوهیدروکسی آپاتیت به همراه مغزاستخوان تزریق و پوست و عضلات بخیه شد.
رادیوگراف از دست خرگوش‌ها بعد از عمل و در روز های 14، 28، 42 و 56 در نمای جانبی تهیه شد. نمونه‌برداری از استخوان جهت ارزیابی هیستوپاتولوژی در هفته هشتم انجام گرفت. رادیوگراف و نمونه‌ها از لحاظ میزان جوش‌خوردگی، فعالیت استخوان‌سازی و دوباره شکل‌پذیری مورد بررسی آماری قرار گرفت.
یافته‌ها: در بررسی رادیوگرافیکی در هفته‌های دوم و چهارم و ششم و هشتم اختلاف آماری معنی‌داری بین گروه‌ها مشهود بود (05/0p<) که این اختلاف در هفته ششم بیشتر شاخص بود. در هفته ششم مشاهده شد که گروه نانوهیدروکسی‌آپاتیت+مغزاستخوان بهتر از گروه نانوهیدروکسی‌آپاتیت به تنهایی عمل کرده بود (02/0p=). در بررسی هیستوپاتولوژیک گروه نانوذرات هیدروکسی‌آپاتیت به همراه مغز استخوان، در مقایسه با گروه کنترل روند استخوان‌سازی از شدت بیشتری برخوردار بود.
نتیجه‌گیری: نتیجه به دست آمده در مطالعه ما بیانگر این است که گروه «نانوهیدروکسی آپاتیت+مغزاستخوان» در مقایسه با دو گروه دیگر «نانوهیدروکسی آپاتیت به تنهایی» و گروه «کنترل» از نظر ترمیم استخوانی بهتر عمل کرده است. پیشنهاد می‌شود این آزمایش با مدل حیوانی بزرگتر انجام شود تا کاربرد بالینی پیدا کند

Effect of Nano-hydroxyapatite Concurrent with Bone Marrow Treatment on Experimental Bone Defect Healing in Rabbit: Radiological and Histopathological Evaluation

Abstract
Background & Aim: Nowadays, bone grafts are used in veterinary and human orthopedic to stimulate fracture healing, accelerate joint connection, and healing of bone defects. In this study, we carried out the radiological and histopathological evaluation of the effects of nano-hydroxyapatite concurrent with bone marrow treatment on rabbit fracture repair.
Materials and Methods: This experimental study was performed in the spring and summer of 2019 on 20 male New Zealand white rabbits (12-month-old, weight=2 kg). The samples were randomly divided into four groups, and bone segments were removed from the radius. In the first group (n=5) (control), the bone defect remained empty and no substance was injected. Meanwhile, the defects in the second, third, and fourth groups (sample size of five in each group) were filled with nano-hydroxyapatite, bone marrow, and combination of nano-hydroxyapatite and bone marrow. Radiographs of the radius in each rabbit were taken at 2nd, 4th, 6th, and 8th post-operative weeks, and bone biopsy was performed in the eighth week for histopathological assessment. In addition, the radiographs and samples were evaluated in terms of union, ossification activity, and re-forming.
Results: In this study, there was a significant difference among the groups in the second, fourth, sixth, and eight weeks regarding radiographical assessments (P<0.05). However, the difference was more evident in the sixth week, when a significant difference was found between the nano-hydroxyapatite group and a combination of bone marrow and nano-hydroxyapatite group (P=0.02). In this regard, the latter had a better performance, compared to the former. In histopathological evaluation, the group of a combination of nano-hydroxyapatite and bone marrow showed higher ossification activities, compared to the control group.
Conclusion: According to the results of the study, the group of a combination of nano-hydroxyapatite and bone marrow had a better function in terms of bone healing, compared to the other two groups. Therefore, it is suggested that the method be applied to larger animal samples to confirm its clinical implications.
