چکیده
مقدمه: استفاده از اتوگرافت همسترینگ جهت بازسازی پارگی رباط صلیبی قدامی زانو، با توجه به نتایج مطلوبی مانند کمکردن موربیدیتی و بهبود روشهای فیکساسیون افزایش یافته است. قطر گرافت نهایی در کاهش احتمال پارگی مجدد مؤثر است. پارامترهای سن، جنسیت، قد، وزن، شاخص توده بدنی(BMI) و اندازه دور ران، به عنوان فاکتورهای پیشبینیکننده قطر نهایی، در مطالعات مختلف گزارش شدهاند؛ اما اجماعی قوی بر سر این موضوع وجود ندارد. همچنین، بررسیهای رادیولوژیک بر پایه امآرآی نیاز به ابزار مخصوص دارد. در این پژوهش پیشبینی قطر تاندون سمیتاندینوسوس پیش از عمل جراحی، در مقایسه با قطر محصول نهایی در حین عمل جراحی، توسط امآرآی مورد آزمایش قرار گرفت.
روش انجام کار: بیماران بالغ با پارگی رباط صلیبی قدامی ثابت شده توسط امآرآی با حداقل کیفیت 5/1 تسلا و تستهای مثبت معاینه بالینی که از اردیبهشت ماه 1396 تا شهریور 1397، به یک درمانگاه آموزشی ارتوپدی مراجعه کرده و کاندید استفاده از اتوگرافت برای بازسازی رباط صلیبی قدامی بودند وارد این مطالعه آیندهنگر مقطعی شدند. تصاویر امآرآی بیماران به نرمافزار Infinite وارد شد و با تعیین سطح مفصلی در نمای کرونال و آنگاه تبدیل آن به نمای اگزیال و سپس اندازهگیری دو فاصله پروگزیمالتر و دو فاصله دیستالتر از سطح مفصل، ماکزیمم قطر تاندون سمیتاندینوسوس بهدست آمد. قطر گرافت سمیتاندینوسوس حین جراحی و نیز قطر گرافت نهایی، اندازهگیری و ثبت شد. از نرمافزار Statistical Package for the Social Sciences (SPSS) ویرایش 22، برای تحلیل دادهها و یافتن همبستگیهای احتمالی استفاده شد.
نتایج: 20 بیمار مرد بالغ، با میانگین سنی 28 سال بررسی شدند. در 4 مورد طول گرافت سمیتاندینوسوس بیشتر از 34 سانتیمتر بود که قطر گرافتهای چهار لا بزرگتر یا مساوی 9 میلیمتر بود. در همه این موارد قطر گرافت سمیتاندینوسوس بیشتر از 6/4 میلیمتر در دو فاصله دیستال به مفصل بود. در 16 مورد دیگر، تاندون گراسیلیس نیز برداشته شد. قطر تاندون سمیتاندینوسوس در دو فاصله دیستالتر از سطح مفصل، مساوی یا بیشتراز 4/3 میلیمتر بود و با قطر گرافت نهایی مساوی یا بیشتر از 8 میلیمتر همبستگی داشت.
نتیجهگیری: روش اندازهگیری پیشنهاد شده میتواند گزینهای قابل اتکا برای جراحان ارتوپد جهت پیشبینی قطر محصول نهایی پیش از عمل باشد.

Pre Operative MRI Diameter Measurement of Semitendinosus Tendon Using for Anterior Cruciate Ligament Reconstruction Compared with the Diameter of the At Surgery

Background: The use of autograft hamstring tendons for ACL reconstruction has increased due to measurable outcomes such as decreased donor site morbidity and improved fixation methods. Final graft diameter is effective in reducing the chance of graft re-tearing. Anthropometric parameters such as age, gender, height, weight, Body Mass Index (BMI) and thigh circumferencehave been reported to be of predictive value in a variety of studies but there is poor consensus agreement. MRI based radiologic assessments also needs radiologic workstation and special software. This study investigated the relationship between pre-operative semitendinosus tendon diameter measured by MRI and intra-operative final graftconstruct diameter.
Methods: Adult patientsconfirmed to have anterior cruciate ligament(ACL)rupture by positive physical test and 1.5 Tesla quality magnetic resonance imaging(MRI),who were candidates for autograft ACL reconstruction entered the study. Those with previous history of ACL surgery or candidates for allograft or bone patellar bone graft reconstruction were excluded. The MRI images of the patients were uploaded into Infinitesoftware. Joint level was determined first in coronal plane and then by using cross linking option the axial joint level was defined. Using cursor and ruler tools the maximum diameters of semitendinosus tendon was measured at the joint line,and two spaces (SPs) proximal and distal to it. Semitendinosus and final graft construct diameters were measured intraoperatively.SPSS software was used to analyze the data to find possible correlations.
Results:TheSemitendinosustendon graft diameter over 3.4 millimeters at two spaces distal to joint line on axial MRI cut well correlated to intra-operative final graft construct diameterof over 8 millimeters.
Conclusion: The proposed measurement techniqueis a reliable option to assist the surgeonsfor pre-operative prediction of graft size in hamstring autograft reconstruction.
