چکیده
پیشزمینه: روش بیحرکتسازی داخلی قطعات شکسته استخوان، مبتنی بر سوراخکاری موضع شکستگی و ثابت کردن آن به وسیله تجهیزاتی است که به استخوان پیچ میشوند. در حین فرآیند سوراخکاری استخوان، این امکان وجود دارد که دما از محدوده مجاز 47 درجه سانتیگراد فراتر رود و آسیب جبرانناپذیر نکروز حرارتی بر جای گذارد. هدف از انجام تحقیق حاضر بررسی فرآیند سوراخکاری استخوان به کمک ارتعاشات آلتراسونیک بود تا مناسبترین شرایط به لحاظ ایجاد حداقل آسیب گرمایی تعیین شود.مواد و روشها: یک مطالعه تجربی به کمک ارتعاشات آلتراسونیک برروی استخوان ران گاو با روش سوراخکاری انجام پذیرفت و میزان دمای ایجاد شده در موضع سوراخکاری بررسی شد. یافتهها: سوراخکاری به کمک ارتعاشات آلتراسونیک در دور rmp ۱۰۰۰ حرارت کمتری در مقایسه با سوراخکاری معمولی ایجاد میکند. در دوران rmp ۲۰۰۰ افزایش دما نسب به "نرخ پیشروی" دریل مستقل میگردد و نرخ پیشروی تاثیرگذار نمیباشد. نتیجهگیری: استفاده از ارتعاشات آلتراسونیک در سوراخکاری استخوان گاو، گرمای کمتر و نکروز استخوانی کمتری در مقایسه با سوراخکاری معمولی ایجاد مینماید و گرمای ایجاد شده به "نرخ پیشروی" وابسته نمیباشد و ممکن است قابلیت بکارگیری در جراحی در انسان را داشته باشد.

Background: Internal immobilization of bone fracture often requires drilling and screw fixation of implanted bone. During bone drilling, the temperature could increase above 47°C causing irreversible thermal necrosis. This study is an experimental bone drilling with ultrasonic-assist in evaluating the temperature increase in comparison with conventional drilling.Methods: An experimental study on a bovine femur specimen was performed by drilling holes with ultrasonic assisted drill. The generated temperature in different speeds was documented at the drilling site, and was compared with the conventional drilling technique.Results: Ultrasonic-assisted drilling at 1000 rpm produced less temperature elevation compared with conventional drilling. Furthermore, at rotational speed of 2000 rpm, the temperature raise was independent of “feed rate” of the drilling.Conclusions: Using ultrasonic vibration in drilling of bovine femur produces less temperature raise in drill site, and works independent “feed rate” when compared with conventional drilling.
