چکیده

پیش‌زمینه: شکستگی اینترتروکانتریک جزء شکستگی‌های شایع در افراد سالمند بالای ۷۰ سال می‌باشد. با وجود آن که تثبیت شکستگی، بهترین درمان است، ولی عوارض آن به دلیل سن بالا و مشکلات همراه بسیار بالا می‌باشد. هدف از این مطالعه، گزارش نتایج درمان جراحی شکستگی‌ اینترتروکاننتریک هیپ بود.مواد و روش‌ها: در یک مطالعه توصیفی آینده‌نگر، کلیه بیمارانی‌که به علت شکستگی اینترتروکانتریک طی دو سال در دو بیمارستان آموزشی اهواز تحت عمل جراحی تثبیت داخلی با روش پیچ و پلاک دینامیک (DHS) قرار گرفته بودند، بررسی شدند. ۸۴ بیمار با میانگین سنی ۷۳/۴ سال (۹۸-۵۹ سال) تحت مطالعه قرار گرفتند که از این تعداد ۳۴  (۴۰/۵%) مرد و ۵۰  (۵۹/۵%)  زن بودند. بیماران حداقل به مدت یک سال مورد پیگیری قرار گرفتند.یافته‌ها: هشت بیمار (۹/۵%) کوتاهی اندام بیش از 2 سانتی‌متر داشتند. ۴ بیمار (۴/۷%) دچار عارضه بیرون‌زدگی وسیله شدند. ۸ بیمار (۹/۵%) دچار بدجوش خوردن شکستگی به صورت بدشکلی واروس، 1 بیمار (۱/۱%) دچار نکروز آواسکولار سرفمور، ۱۳ بیمار (۱۵/۴% ) عفونت عمل، و 2 بیمار (۲/۳%) دچار بدجوش‌خوردگی شکستگی محل شدند. ۹ بیمار (۱۰/۷% ) طی ۲۴ هفته فوت کردند. ۱۰ بیمار (۱۱/۹%) تحت عمل جراحی و تثبیت مجدد قرار گرفتند. میانگین زمان برگشت به فعالیت قبل از عمل ۵ هفته بود. نتیجه‌گیری: عوارض شکستگی‌های اینترتروک هیپ، حتی پس از تثبیت با پیچ و پلاک بالا می‌باشد. به جز درصد بالاتر عفونت، سایر یافته‌ها با مطالعات مشابه در پیشینه تحقیق، هسمو است.

چکیده [English]

Background: Intetrochanteric hip fracture commonly affects over 70 years old people. While the best acceptable treatment is early fixation, the complication rate is fairly high because of patients' older age and associated age-related comorbidities. We would like to report our experience with the outcome of fixation of such fractures in our locality.Methods: In a descriptive study all the intertrochanteric fractures treated with compression screw-plate system during a 2 year period in two hospitals of Ahwaz were studied. 84 patients with mean age of 73.4 (range of 59 to 98 years) were assessed. 34 (40.5%) male and 50 (59.5%) female constituted the study group. Patients with a minimum of one year follow-up were evaluated.Results: The complications consisted of 8 (9.5%) varus, 8 (9.5%) shortening, 4 (4.7%) fixation "cut-out", 1 (1.1%) AVN, 13 (15.4%) wound infection and 2 (2.3%) non-:::union:::, 9 (10.7%) mortality in 24 weeks. 10 (11.9%) underwent re-operation and re-fixation. The average time to returning to pre-fracture activity was 5 months.Conclusion: Complication rate is relatively high in intetrochanteric hip fractures even after screw-plate fixation. Except for high infection rate, in our series, the overall results are similar to the other reports in the literature.
