Background: Ankle arthrodesis is one of the major surgeries for the treatment of advanced osteoarthritis of the ankle joint. There are various techniques available for ankle arthrodesis and each technique has unique advantages and disadvantages. The aim of the present study was to evaluate the results of ankle arthrodesis with two this different approaches.Methods: The medical records of the patients who were admitted for ankle arthrodesis and treated by a single surgeon from January 2015 to January 2020 were reviewed. The patients were recalled for re-evaluation after a mean follow-up of 19 months. Functional outcomes were assessed using validated AOFAS Ankle-Hind foot score, Manchester-Oxford Foot Questionnaire (MOXFQ) and visual analog scale (VAS) score for pain. The collected data were recorded in a pre-designed checklist and then analyzed using SPSS software.Results: A total of 32 patients, 18 men and 14 women with a mean age of 46.7 years were included in the study. 67.6% had history of ankle fractures. Anterior approach was used in 24 patients, 75%, and the rest of the patients had surgery with lateral approach (25%). The most common instrument for arthrodesis was concomitant use of plate and screws in 18 patients (56%). Union occurred in 28 patients (87.5%) within 11.1 weeks after the surgery. The AOFAS score increased significantly and MOXFQ and VAS scores decreased significantly following the surgery (p<0.001 for each). Advanced age and intramedullary nail were related to prolonged time to union (p<0.05). Surgical approaches (either anterior or lateral) had no association with AOFAS, MOXFQ, and VAS scores. Non-union was seen in 4 patients (12.5%) and deep infection in 3 patients (9.3%) after ankle arthrodesis.Conclusion: The study results showed that, this surgery could improve patients’ pain and function with relatively low post-operative complications.

Introduction: According to the literature, cardiovascular diseases are highly prevalent globally, while there is an increased risk of cardiovascular-related death in osteoarthritic patients. Therefore, the present study intended to investigate the cardiovascular risk factor prevalence in osteoarthritic patients.Methods: This cross-sectional, descriptive study was performed at the orthopedics clinics of the Guilan University of Medical Sciences in 2017. The inclusion criteria were the 40-75-year-old patients who were selected using the convenience sampling method. They were diagnosed with osteoarthritis, and gave informed consent for participation. The study data included demographics, cardiovascular risk factors, medical history, and anthropometric measurements. After a qualitative analysis, the data were analyzed using the SPSS software version 16.Results: A total of 100 patients with knee osteoarthritis, including 79 female and 21 male patients with the mean age of 53.9±11.9 years, were included in the study. The prevalence of diabetes, hypertension, obesity, metabolic syndrome, and smoking in the participants was 31%, 33%, 45%, 51%, and 9%, respectively. 12% reported a history of previous heart attacks. Moreover, the prevalence of diabetes, obesity, hypertension, and metabolic syndrome was higher in women than men. Also, the mean age of the male patients was significantly higher than women (P = 0.03).Conclusion: Given the high prevalence of cardiovascular risk factors in osteoarthritis patients, it is essential to notice the early cardiovascular disease detection in patients with knee osteoarthritis, especially in female patients.

Background: Osteoarthritis is a major public health consideration which leads to disability because of chronic pain, stiffness, sleeping disorder and depression. Among the herbal products Psidium Guajava L. fruit could be regarded as a potential natural remedy due to its anti-oxidant characteristics. The aim of this study was to evaluate the healing effect of Psidium Guajava L. fruit on osteoarthritis of the knee in rats.Methods: Forty male Sprague-Dawley rats with induced osteoarthritis by intra-articular injection of 500U of type 2 collagenase in the left knee were recruited in four treatment groups (n=10): (A) control, (B) Piascledine (10mg/kg) as positive control, (C) oral administration of hydro-alcoholic extract of Psidium Guajava L. fruit (500mg/kg), and (D) oral administration hydro-alcoholic extract of Psidium Guajava L. fruit (1000mg/kg). After 8 weeks of daily administration, the outcome was evaluated by histopathological and radiographic assessments. The data were analyzed using Kruskal-Wallis nonparametric in SPSS version 23.00, and P<0.05 was considered statistically significant.Results: Psidium Guajava extract exhibited good radical scavenging activities with IC50 of 0.46 ± 0.13 mg/mL (quercetin as a positive control). Histopathological assessments of the cartilage showed smooth and continuous articular surface with columnar cell distribution in the experimental group that received 1000mg/kg Psidium Guajava L. fruit hydro-alcoholic extract which healed better than others, especially in the control group (p-value =0.02). The radiological assessments of the knee joints showed similar findings in histopathological assessment. Conclusion: Psidium Guajava L. fruit hydro-alcoholic extract might be regarded as an effective complementary and alternative treatment for osteoarthritis of the knee in rats.

Osteoarthritis is a chronic systemic disease that can involve any joint in the body, but more often lower limb bearing joints and hand joints. The prevalence of this disease in humans increases with age and obesity. Biomechanical and biological changes that occur after knee injuries are involved in the development of post-traumatic osteoarthritis and, as a degenerative joint disease, cause irreversible hyaline cartilage destruction; which reduces the quality of life by causing permanent pain and joint dysfunction.
In order to better understand how osteoarthritis progresses, it needs to be modeled in animals; especially in the early stages of the disease, as well as the evaluation of the effects of new drugs and therapies. These models can be spontaneous or inductive. Spontaneous osteoarthritis can occur naturally in genetically engineered strains of rats. Intra-articular injection of chemicals such as mono-iodiacetate can also induce osteoarthritis. Today, many surgical modelings are also used to induce osteoarthritis, all of which indicate joint instability to cause the disease. Guinea pigs are commonly used for research on articular cartilage and are common models for studying the reconstruction process of focal cartilage defects. The development of experimental conditions inducing osteoarthritis is similar to the natural course of the disease in human due to joint instability and the final gross pathology is exactly the same.
The recent used methods to modeling osteoarthritis include the removal of lateral meniscus and anterior cruciate ligament transection, which appear to reduce inflammation and only create a mild joint instability; that way not only creates repeatable lesions in the model, but also the same kind as the lesions of osteoarthritis in humans. This method allows for the evaluation of cartilage degeneration by radiography, histology and molecular analysis.

The goals of scientific research of this study were to the evaluate the regeneration articular cartilage effect of injection of the hydroalcoholic extract of leave of Psidium Guajava with Stem Cell into the knee joints of rats.
Methods: Firstly, ninety male rats were choosen, and collagenase type II was prepared for an intra-articular knee injection, for them to induce OA in the animal models. Then, they were divided into nine treatment groups. (N = 10): (a) Sham, (b) Control, (c) injection of hyaluronic Acid intra-articular, (d) Injections of Stem Cell intra-articular, (e) Injections of PRP, (f): Injections of the extract Psidium guajava leaves (g) Injections of the extract Psidium guajava leave with PRP, (h) Injections of the extract Psidium guajava leaves with Stem Cell (i) Injections of the extract Psidium guajava leaves with Stem Cell and PRP. After 5 months, the results were assessments by a Radiologist and Histopathologist.
Results: Radiological and histopathological findings were in which the improvement of the articular cartilage was better in the leaves with Stem Cell and PRP compared with the control group(P<0.05).
Conclusions: It seems that Psidium guajava leaves can be suggested as an effective complementary and alternative treatment for OA of the knee. Because it has a lot of polyphenol compounds.

Background: Osteoarthritis is a progressive joint disease. Mastic fruit has antioxidant and anti-inflammatory properties. Bone marrow stem cells have the ability to differentiate into chondrocyte. The purpose of this study is evaluation the effect of Pistacia atlantica hydro alcoholic extract with bone marrow- derived stem cells on induced knee osteoarthritis in rat.
Methods: Firstly, 50 male Wistar rats were divided into 5 groups of 10. Then, on days zero and 3, fifty μl collagenase type 2 under anesthesia was injected in the left knee. After 10 weeks, osteoarthritis was induced and rats based on the type of treatment were divided into 5 groups including: control, treatment by stem cell and Pistacia atlantica hydro alcoholic extract, Pistacia atlantica hydro alcoholic extract therapy, stem cell therapy and treatment with hyalgan. At the end of study, radiographic and histopathology surveys and also measured of antioxidant properties as MDA-TAC was done.
Results: After 12 weeks of treatment, radiographic and histopathological evaluation showed that the results of the cell with extract group was significantly different compared to other treatment groups and the control group (P <0.05). Also, based on biochemical evaluation of MDA-TAC in cell with extract group significant reduction in oxidative stress is observed compared to other treatment groups and control (P <0.05).
Conclusions: It seems that Pistacia atlantica hydro alcoholic extract with bone marrow- derived stem cells are effective in knee cartilage repair.

Background: Knee osteoarthritis is a common etiology of disability in older age group. There are evidences suggesting that varus or valgus deformity can be from distal of femur, proximal of tibia or because of ligamentous laxity. Osteotomy in incorrect location may create zigzag deformity and may contribute to increase risk of osteoarthritis. The objective of this study was to find the correct location of deformity in genuevarum or genuevalgum.Methods: We studied 40 patients with knee deformity at a mean age of 27.4 years (19-38), prospectively. Standing 3-point view radiographs were obtained on each patient, and the limb axis was determined. The data on limb malalignment was analyzed statistically.Results: In 94.5% of cases the location of varus deformity was proximal of tibia and in 5.5% in distal of femur, the best location of osteotomy. Our study also revealed multiple factors contributing to varus deformity, i.e. distal femur in 67.5%of cases, proximal tibia deformity in 92.5% of cases and ligamentous laxity in 77.5%.Conclusions: Varus knee deformity can be from proximal tibia, distal femur or knee laxity. Before any corrective surgery one must clarify location of deformity

Background: Osteoarthritis (OA) is a degenerative disease of the joints. The asporin (ASPN) gene encodes a cartilage extracellular protein belonging to the small leucine-rich proteoglycan family. Polymorphisms in the aspartic acid (D) repeat are associated with OA susceptibility. The D14 allele is associated with increased OA susceptibility in the Japanese and the Han Chinese but is not an important factor in OA etiology among Caucasians, though the D15 allele is a risk factor for the Greek population. In this study, the effect of ASPN on Iranian with knee OA is investigated.
Methods: 100 knee OA patients (72 female, 28 male) in a training hospital in Tehran, Iran were compared with 100 controls with no sign of OA, and the allelic association of the D-repeat polymorphism was studied in two groups.
Results: There were some minor differences in the frequencies of the D14 and D15 alleles between patient and control groups. These differences were, however, significant only for females. In Iranian females, D15 allele was significantly associated with KOA (p < /em>=.045), and D14 allele was significantly protective from KOA (p < /em>=.032).
Conclusions: D15 allele could be considered a risk allele only for women (p=.045, OR=1.73, 95% CI=1.01-2.94) in the Iranian population. This association is in part similar to that finding for the Greek population.

Background: Knee osteoarthritis is a common joint disorder in the older age patients. This study was performed to compare the efficacy of injection of methyl-prednisolone versus sodium-hyaluronate in symptomatic mild to moderate knee osteoarthritis.Methods: In a randomized clinical trial, 53 patients with knee osteoarthritis were assigned to receive either methyl-prednisolone (26 patients) or sodium-hyaluronate (27 patients) in a hospital in Tehran, Iran. Sodium-hyaluronate three injections within 2-weeks interval and a single injection of 40 mg of methyl-prednisolone were given. The improvement rate was compared after one and six months.Results: The total score significantly improved in both groups (p < /em>=.001). There was no significant difference between the two groups after the first month (p < /em>≥.05) but the results were better after six months in sodium-hyaluronate injection group (p < /em>=.001).Conclusions: Intra-articular injection of sodium-hyaluronate in comparison with methyl-prednisolone has longer efficacy in treatment of symptoms of mild to moderate knee osteoarthritis.

Background: Knee dislocation, although a rare injury, is a severe injury which can endanger the limb viability or cause a significant impairment in the knee function. The purpose of this study was to evaluate the knee function and knee osteoarthritis in the patients who had sustained an acute traumatic knee dislocation . Methods: In a retrospective study, 19 consecutive patients with knee dislocation who were treated by different therapies were studied during 5 years in a teaching hospital in Urmia, Iran. Vascular injuries lead to amputation in 3 patients. The follow-up study was performed on the remaining 16 patients within 37±13 months. The patients were evaluated for knee stability, range of motion and the "Tegner-Lysholm" knee function scores. The development of knee osteoarthritis was classified according to Kellgren and Lawrence classification . Results: The knee function was excellent in one, good in 3, fair in 9, and poor in 3 cases. The younger patients and knees with wider range of motion had better "Tegner-Lysholm" scores. The knee scores had significant correlation with the severity in the class of dislocation. The time of surgical reconstruction, whether early or delayed, had no significant effect on the outcome. The mean knee range of motion for the 12 patients who were treated surgically was 120±15 degrees and for the 4 patients who were treated non-surgically was 115±28 degrees. There was a significant correlation between the "Tegner-Lysholm" scores and knee range of motion. Conclusions: Knee dislocation is a very severe complex trauma and normal knee functional outcome is not often achieved. The knees with the more extensive ligament injuries have less favorable outcomes. Younger patients and knees with larger range of motion had better functional outcomes.

Background: There are different conservative treatments for the osteoarthritis of the medial compartment of the knee and valgus bracing is considered as important one. The purpose of current study was to examine the efficacy of the brace unloading in changing floor reaction force.Methods: Ten patients with osteoarthritis in medial compartment of the knee contributed in this quasi-experimental study with and without using knee braces. The magnitude of the mediolateral vector of the ground reaction force and center of pressure displacement in mediolateral direction were measured by using a force plate system (Kistler, Swiss). Statistical tests were used to analyze the data.Results: The mean of displacement of center of pressure (COP) was 25 mm and 5 mm with and without brace, respectively (p < /em>≥.05). The mean of mediolateral vector of the ground reaction force was 28.17 with and 22.45 without brace and there was no significant difference (p < /em>≥.05). In contrast, there was a significant difference between the magnitudes of the mediolateral vector of the ground reaction force within pre-and post-brace situations (p < /em>=.000).Conclusion: Valgus bracing can reduce the magnitude of the mediolateral vector of the ground reaction force and displacement of the COP in the mediolateral direction. It might be concluded that this method can reduce the pain and instability and improve function in patients with osteoarthritis of the medial compartment of the knee.

Background: Mechanical articulated devices can replace the intervertebral disc in patients with chronic back degenerated pain. We would like to report our short-term experience with a disc prosthesis in a small group of L5-S1 disc degeneration.Methods: In a cohort study, 21 consecutive patients (17 males, 4 females) with degenerative L5-S1 disc disease with over 6 months history of disabling back symptoms non-responsive to conservative treatment were treated by disc prosthesis. The mean age was 36±5.7 years (22-48 years) at surgery and the average follow-up was 11 months (13-24 months). Background: Osteoarthritis is common especially in over 50 years of age patients. There are several non–surgical treatment modalities such as non-steroid anti-inflammatory drugs, physiotherapy, and intra-articular injection of steroids and Hyaluronic Acid. Surgical procedures include arthroplasty, osteotomy of tibia and arthroscopic débridement. Careful selection of patients in particular, for arthroscopic débridement is a very important point.Methods: In a prospective study, 88 patients with mean age of 55.28 (55-63 years) with knee osteoarthritis who had failed non-surgical treatments were selected for arthroscopic débridement. Radiographic inclusion criteria were presence of 3-4 millimeters of joint space, less than 10 degrees flexion contracture and varus/valgus malalignment, and at least 100 degrees of flexion. The treatment results were assessed annually in 3 successive years, using Lysholm knee Scale. Seventy-four patients (43 females, 31 males) had complete follow-up and are reported here.Results: The pre operative Lysholm score of 37.2 (range: 26-55) increased to 81.9 (range: 70-90) in first year, to 82.9 (range: 70-95) by the second year and to 78.5 (60-90) in the third year (p < /em>

Background: Nativization is modification of the components of a system in accordance with the needs of a society without altering its original function. We hypothesized that an index that incorporates Iranian society native specifications would be more sensitive in measurement of hip and knee osteoarthritis severity in comparison to common international indices.Methods: Thirty patients with primary osteoarthritis of hip or knee that underwent total joint arthroplasty and fulfilled the criteria were enrolled. Patients were studied using WOMAC, Lequesne, Modified Doyle index, nativized WOMAC index and patient and interviewer global assessment preoperatively and also 6 weeks, 3 months and 6 months post-operatively. "50 feet walking time" and knee range of motion were also studied. The results were analyzed using parametric and nonparametric tests and Pearson coefficient of correlation.Results: The nativized index improved from mean 118 (99-136) pre-operatively to 53.4 (41-68) at 6 weeks post-operatively and 31.5 (23-44) at 6 months. WOMAC and Lequesne improved from mean pre-operative values of 75.6 (55-92) and 18.6 (11-24) to 18.8(17-24) and 6.6 (3-11) at 6 months. Doyle index also improved significantly. Interviewer and patient global assessment improved from mean pre-operative value of 3.4 and 3.9 respectively to 1.5 and 1.9 at 6 weeks and 0.7 and 0.8 at 6 months post-operatively respectively. The most sensitive indices were nativized WOMAC and Lequesne indices.Conclusions: Nativized WOMAC index in knee and hip osteoarthritis demonstrates the severity of symptoms with more sensitivity than other common international indices.

Background: Osteoarthritis is a relatively common ailment of human beings. The aim of this study is to compare the short-term effects of oral non-steroid anti-inflammatory medications with intra-articular injections of Hyaluronic Acid or Methyl Prednisolone acetate.Methods: In a clinical trial study, 40 female patients with moderate or severe knee osteoarthritis received Hyaluronic Acid intra-articular injections. They were compared with 16 cases who had intra-articular Prednisolone injection and 36 female patients who received non-steroid anti-inflammatory oral medication. The results were evaluated in 6 and 24 weeks by Visual Analogue Scale and Western Ontario & McMaster Universities (WOMAC) functional score.Results: Pain relief in 6 and 24 weeks post injection were 4.9 and 4.7 for Hyaluronic Acid, and 1.5 and 1.6 for Prednisolone. The figures for oral non-steroidal anti-inflammatory medication were 5.2 and 4.9 respectively. Subjective patient’s satisfaction was expressed in 28 (70%) of Hyaluronic Acid group and 16 (100%) of Prednisolone group.Conclusions: After 6 and 24 weeks following intra-articular steroid injection in female patients with knee osteoarthritis, pain relief and subjective patient satisfaction is more favorable compared to intra-articular Hyaluronic Acid injection. These two are both more effective than oral anti-inflammatories. This statement does not include the potential side-effects of intra-articular steroid injection in long-term.
