Background: Hip hemiarthroplasty is a common method in treating femoral neck fractures in patients older than 70 years old. Depending on the severity of bone destruction, and bone stock, two methods either cemented or non-cement bipolar prosthesis may be used.Methods: In a prospective study, 60 patients with femoral neck fractures who were referred to the university hospitals of Isfahan, Iran, were divided into two treatment groups: 30 patients were treated with cemented bipolar method and 30 with non-cement bipolar hip arthroplasty surgery. In a 6-month follow-up, the surgical outcomes were assessed using the VAS pain scale, SF36 general health instrument score, and Harris hip score (HHS). Results: The pain scale (VAS), SF36 and Harris scores showed non- significant differences in the two groups: pain=3.63 out of 10 in 3 months and 2.6 out of 10 in 6 months; SF36= 69 and 86 in 3 and 6 months ,respectively; HHS=78 in both in 3 and 6 month in the cemented group .In non-cemented group the pain scores were 3.91and 3.12 in 3 and 6 months; SF36=67 and 83, and HHS=82 in 3 and 6 months respectively.Conclusion: It seems that the two methods of cemented and non-cemented bipolar hemiarthroplasty, in elderly hip fractures, would have similar pain, quality of life, and hip function in short term follow-ups.

Background: Hip fracture is very common orthopaedic fracture in elderly. The present study investigates the outcome after surgical treatment.Methods: The hip fractures of over 65-year old patients that had undergone surgery in a 5-year period were retrospectively studied in a hospital in Semnan, Iran. The information of 259 patients (120 male, 139 women) were collected including demographic data, medical history, history of surgery (type of anesthesia, technique of surgery), length of hospitalization before and after surgery, and death in hospital.Results: From all the operated cases, 174 (67.2%) received spinal, 68% (26.3), epidural and 17% (6.6) general anesthesia. 185 pateints (71.4%) had no complication and 74% (28.6) had at least one complication. 12 patients (6.4) died while in hospital. 56 cases (21.6%) were walking with no problem 32 (12.4%) required walking and 9 patients (3.5%) were not able to walk.Conclusions: Complications after surgery for hip fracture are relatively high. To recover faster and reduce more serious complications, it is recommended to change the strategies on how to care for the elderly during and after surgery.

Background: Femoral neck fracture is uncommon but serious injuries in young adult with high complication rate. This study was designed to evaluate the outcome of surgical treatment of such a fracture fixed by screw in. Methods: In a descriptive prospective study form 2006 to 2009, 42 patients with the mean age of 37 years (15–60 yrs) were treated by closed or open reduction and screw fixation for femoral neak fractures in two university hospitals in Ahvaz- Iran. They all received surgery within the first 24 hours after injury. They were followed for 15 months on average. Results: In accordance with Garden classification, 2 patients were type I, 3 type II, 18 type III and 19 type IV Gorden fractures. Thirty two cases were fixed by open reduction and fixation and 10 by closed reduction. :::union::: was achieved in 30 pateints. The 12 (28.6%) cases of non:::union::: were seen in the ones who received open reduction and fixation. Avascular necrosis was observed in 7 patients (16.7%), 4 of whom had obtained :::union:::. Conclusion: Femoral neck fracture is associated with high complication rate, in particular, if open reduction becomes necessary.
