چکیده
پیش زمینه: بیشتر صاحبنظران در مورد لزوم درمان جراحی شکستگیهای ناپایدار مهرههای سینهای و کمری توافق دارند، اما درمورد روش جراحی پشتی یا از جلو، هنوز اتفاقنظر کامل وجود ندارد. در این تحقیق نتایج سوبژکتیو و ابژکتیو جراحی پشتی با بهکارگیری یک سیستم سگمنتال و دکمپرس کردن کانال نخاعی از طریق لیگامنتوتاکسی بررسی شد.مواد و روشها: در یک مطالعه طولی ۳۷ بیمار بین فروردین ۱۳۸۰ تا فروردین ۱۳۸۴ در یک مرکز درمانی شیراز تحت درمان جراحی قرار گرفتند. شکستگی مهرههای سینهای و کمری به مدت میانگین ۳۰ ماه (۴۶-۱۲ ماه) بعد از عمل جراحی پیگیری شدند. در پایان پیگیری خصوصیات سوبرکتیو و ابژکتیو شامل درد کمر (مقیاس نمرهبندی دنیس)، سطح عملکرد، وضعیت شغلی (مقیاس شغلی دنیس)، وضعیت عصبشناسی (درجهبندی فرانکل) و شاخصهای پرتونگاری (زاویه کایفور، زاویه گوهای، کلاپس، شاخص ساژیتال، زاویه پشتی مهره و شکستن یا جابهجایی وسیله) مورد بررسی قرار گرفتند..یافتهها: بیست بیمار درد نداشتند، ۳۵ بیمار عملکرد مستقل داشتند (درجه FIM ،F7 ،F6) و بیمارانی که ضایعه عصبی داشتند ۱/۶ درجه فرانکل بهبودی نشان دادند. میانگین زاویه کایفوز (موضعی) از ۱۹/۱ درجه قبل از عمل به ۱/۱ درجه بعد از عمل و ۱/۵ درجه در پیگیری نهایی رسید. عوارض جراحی محدود به ۳ مورد عفونت خفیف موضعی بود و فقط یک مورد شکستن وسیله مشاهده شد.نتیجهگیری: از نظر تصحیح معیارهای پرتونگاری شکستگی و نتایج سوبژکتیو و آبژکتیو، این روش یک شیوه سریع، موثر و ایمن میباشد، جراحان با آن آشناترند، عوارض کمتری نسبت به جراحی از جلو دارد و میزان شکستن یا جابهجایی وسیله نیز در آن اندک است.

Posterior Segmental Instrumentation and Fusion of Thoracic and Lumbar Vertebrae Fractures (Short-Term Outcome)

چکیده [English]
Background: Most investigations agree that unstable thoracolumbar fractures require surgical treatment. There is, however, debate on the best approach-anterior, posterior or combined. We are reporting our short and mid-term results on a «posterior only» approach with ligamentotaxis-dependent canal decompression with a segmental instrumentation system.Methods: 37 consecutive patients with thoracic and lumbar vertebrae fractures over 4 year period, with a mean follow-up of 30 months, (12-46 months) were studied. The cases underwent subjective and objective evaluation, using the Denis back pain scale, functional independent measure, employment status, neurologic Frankel grading and radiographic indices of vertebral angles and height change.Results: Twenty patients were pain-free, 35 were functionally independent. Frankel neurological grading showed 1.6 points average recovery. The mean preoperative kyphoitc angle of 19.1º improved 1.1º post surgery and to 5.1º at final follow-up. There were 3 minor infections and one hardware failure.Conclusions: Posterior spinal decompression and instrumentation for unstable thoracic and lumbar fractures is safe, effective and is associated with effective, achievement of radiographic correction, and is easier than anterior approach.
