چکیده
پیشزمینه: روش استاندارد ترمیم یک عصب محیطی، ترمیم با بخیههای اپینورال و با بخیههای مجزا میباشد. در این مقاله روشی توضیح داده شد که با کمک آن عصب با بخیههای پیوسته ترمیم میشود و هدف مقایسه این روش با روش استاندارد بخیههای مجزا بود.
مواد و روشها: در یک کارآزمایی بالینی، ۲۵ سگ مورد مطالعه قرار گرفتند. عصب سیاتیک سگها تحت بیهوشی عمومی با چاقوی جراحی شماره ۲۵ بهصورت کاملاً تیز بریده شد. سگها بهصورت تصادفی در یکی از سه گروه شاهد (۵ سگ)، ترمیم عصب سیاتیک بهصورت بخیههای مجزا (۱۰ سگ) و ترمیم عصب با بخیههای پیوسته (۱۰ سگ) قرار گرفتند. در گروه شاهد عصب بدون دوخته شدن رها شد. پس از ۶ هفته سگها کشته و عصب با میکروسکوپ نوری و الکترونی بررسی شد. میزان نخ مصرفی، زمان ترمیم عصب، قطر میلین و قطر آکسون مورد بررسی قرار گرفت. در نمای فراساختاری شواهد دژنراسیون و رژنراسیون جستوجو شدند.
یافتهها: زمان ترمیم و میزان نخ مصرفی با تفاوت معنیدار آماری در گروه ترمیم پیوسته، کمتر بودند (۰.۰۰۱=p). از نظر یافتههای میکرسکوپ نوری تفاوتی بین دو گروه مشاهده نشد و شواهد رژنراسیون در میکرسکوپ الکترونی فقط در گروه ترمیم پیوسته مشاهده گردید.
نتیجهگیری: نتیجه روش شرح داده شده در مقاله حاضر، مشابه روش استاندارد است. اگرچه این روش بدون شک معایبی دارد ولی میتواند در موارد زیادی جایگزین روش استاندارد ترمیم عصب شود.

Repair of Peripheral Nerve Injury with Continuous Epineural Suturing (An Animal Study)

Background: The
standard method for repair of an injured peripheal nerve is epineural repair
with separate sutures. In this article, we described a method of continuous
running epineural suturing in an animal model, and compared
it with the standard interrupted technique. Methods: In a clinical trial study, the sciatic nerve of 25 dogs was cut by a sharp blade
under general anesthesia. The dogs were randomly divided in 3 groups. The 10
dogs received simple interrupted suturing of the cut sciatic nerve (control
group), another 10 had continuous suturing technique, and the remaining 5 were
left unrepaired. After 6 weeks the dogs were sacrificed and the nerves were
studied by light and electron microscopy. The amount of consumed suture
material, time of repair, myelin thickness and axon diameter were examined.
Ultrastructural studies were performed to assess the degeneration and
regeneration process. Results: The time of suturing and the amount
of consumed suture material were significantly lower in the continous group (p < /i>
