Two-stage grafting with preparation of a smooth bed before grafting is a technique used for tendon or bone defects in the past. We are now reporting such a technique for large scarred gaps in the sciatic nerves of rat.Methods: In each of the 20 female Wister rats, 15 centimeters of the sciatic nerve was excised. The nerve bed was then covered by tetracycline powder to induce scar tissue formation. In half of the rats a silicone tube was sutured into the nerve ends to produce a smooth tunnel for a two-stage grafting. In the second group the cut nerve ends were covered by silicone caps. After 4 weeks each rat received nerve grafting of its sciatic nerve defect from its bilateral median nerves. 15 weeks following the grafting the rats were tested for their sciatic nerve functions, then the nerves were excised for histological evaluation of the nerve regeneration and the scar tissue.Results: The motor and sensory motor function recovery of the nerves, as measured by “External Postural Thrust”, and “Withdrawal Reflex Latency”, were greater in the group that had the grafts replacing the silicone tubes. The histological evaluation also confirmed the presence of more nerve fibers and less scarring in the silicone tube treated group.Conclusions: Two-stage nerve grafting in severely scarred bed can improve the functional results of a nerve graft.}, keywords = {Graft,Scar,Sciatic,Nerve}, title_fa = {نتایج گرافت عصبی در بستر اسکار در عصب سیاتیک موش صحرایی (مقایسه روش یک مرحله‌ای و دو‌‌ مرحله‌ای)}, abstract_fa = {پیش‌زمینه: با وجود تکنیک‌های متعدد برای درمان آسیب عصبی در بستر اسکار، نتایج این درمان‌ها هنوز رضایت‌بخش نمی‌باشد. تکنیک پیوند دومرحله ای در مورد آسیب‌های تاندونی گزارش شده است. در این مطالعه روش دو مرحله‌ای پیوند عصب در مورد آسیب با فاصله بلند در عصب سیاتیک در بستر اسکار آزموده شد.مواد‌ و روش‌ها: در ۲۰ موش صحرایی، عصب سیاتیک پای راست بریده و خارج شد. موش‌ها به دو گروه «پیوند یک مرحله‌ای» و «پیوند دو مرحله‌ای» که عبارت بود از گذاشتن گرافت در کانال ایجاد شده با یک میله سیلیکون تقسیم شدند. در گروه ۱ لوله سیلیکونی در محل عصب به دو سر عصب پیوند شد و در گروه ۲ روی سر عصب کلاهک سیلیکونی قرار داده شد. در دو گروه ایجاد بافت اسکار در بستر عصب توسط پودر تتراسیکلین القا گردید. پس از چهار هفته، در تمام موش‌ها طی عمل دوم، از عصب مدیان دو دست برای پیوند به عصب سیاتیک استفاده شد. ۱۵ هفته پس از پیوند، تمامی موش‌ها تحت ارزیابی‌های عملکردی و بررسی هیستولوژیک اسکار و برش‌های عصبی قرار گرفتند.یافته‌ها: عملکرد حرکتی و حسی که توسط آزمون‌های «زاویه کف پا با راستای حرکت»، «مقایسه قدرت عضلات اکستانسور کف پا»، «نسبت وزن‌تر عضلات سولیوس و گاستروکنمیوس» و «وقفه پاسخ گویی به تحریک حسی» سنجیده شدند، در گروه ۱ پیوند دومرحله‌ای نتایج بهتری نشان داد (۰/۰۵>p < /em>). نتایج بافتی نیز با این نتایج همخوانی داشتند و میزان اسکار کمتی را در گروه ۱ نشان داد.نتیجه گیری: این مطالعه توصیه می کند که پیوند دو مرحله ای عصب در بستر اسکار موجب بهبود نتایج پیوند عصب می شود. مطالعات بیشتری برای بررسی امکان استفاده از این روش در انسان مورد نیاز است.}, keywords_fa = {گرافت,اسکار,عصب سیاتیک}, url = {https://www.ijos.ir/article_121237.html}, eprint = {https://www.ijos.ir/article_121237_9ce4cee370a496ca40fd3948423248dc.pdf} }
