New technology, however, masks the importance of history and physical examination. This study aims at evaluating the accuracy of physical examination and MRI scanning in diagnosis of knee injury, including meniscal tears and cruciate ligaments ruptures.Methods: In a cross-sectional, analytic-descriptive study, 120 patients (108 males, 12 females) with knee injury who were candidates for arthroscopy were recruited in a Hospital in Tabriz during a one-year period. Prior history of arthroscopy or knee surgery was considered as exclusion criteria. A complete history and physical examination was performed and diagnosis was documented before looking at the MRI results. The final arthroscopic diagnosis was then compared with clinical and MRI studies.Results: 120 knee injury cases with a mean age of 29 (16-54) years were enrolled. Accuracy, sensitivity and specificity of clinical examination was respectively 85, 94.8 and 75.8 percent for medial meniscus, 85, 70.8 and 88.5 percent for lateral meniscus, 95.8, 98.6 and 91.7 percent for anterior cruciate ligament and 100 percent for posterior cruciate injuries. Accuracy, sensitivity and specificity of MRI were respectively 77.5, 84.2 and 71.4 percent for medial meniscal. 85.8, 56.5 and 92.8 percent for lateral meniscal, 92.5, 98.6 and 83.3 percent for anterior cruciate, and 100 percent for posterior cruciate injuries. Overall, in isolated injuries, the accuracy of clinical examination was relatively better than MRI in complex injuries, while MRI had higher accuracy in complex intra-articular injuries.Conclusion: According to our results, both physical examination and MRI scanning are very sensitive and accurate in diagnosis of knee injuries, with a slight preference of physical examination. MRI should be reserved for doubtful or complex injuries.}, keywords = {Arthroscopy,Magnetic resonance imaging,Menisci, tibial,Ligaments, articular}, title_fa = {بررسی صحت معاینه فیزیکی در مقایسه با ام‌آرآی در تشخیص آسیب‌های رباطی داخل زانو با آرتروسکوپی}, abstract_fa = {پیش‌زمینه: تصویربرداری با کمک تشدید مغناطیسی (MRI) از زانو معمولا به‌عنوان جایگزین غیرتهاجمی آرتروسکوپی تشخیصی درنظر گرفته می شود. از نظر بالینی، ام‌آرآی جهت تایید تشخیص آسیب منیسک یا رباط پیش از بررسی آرتروسکوپیک و جراحی بکار می‌رود. از سوی دیگر، این فن‌آوری گاهی از اهمیت اخذ شرح حال و معاینه بالینی می‌کاهد. هدف از این مطالعه بررسی دقت معاینه بالینی و ام‌آرآی در تشخیص آسیب زانو شامل پارگی‌های منیسک و رباط‌های متقاطع است.  مواد و روش‌ها: در یک مطالعه مقطعی از نوع توصیفی‌ـ‌ تحلیلی، ۱۲۰ بیمار (۱۰۸ مرد و ۱۲ زن) دچار آسیب زانو که سابقه قبلی آرتروسکوپی نداشتند و کاندید آرتروسکوپی بودند، در مدت یک سال در یک مرکز درمانی تبریز بررسی شدند. ابتدا برای تشخیص اولیه، معاینه دقیق و کامل زانوی مبتلا انجام شد. نتایج آرتروسکوپی زانو به‌عنوان تشخیص نهایی درنظر گرفته شد و بر این اساس نتایج معاینه فیزیکی و ام‌آرآی مورد قضاوت قرار گرفت.یافته‌ها: میانگین سنی بیماران ۲۹ سال (۵۴-۱۶سال) بود. دقت، حساسیت و ویژگی معاینه فیزیکی در مورد آسیب منیسک داخلی به‌ترتیب ۸۵، ۹۴/۸ و ۷۵/۸درصد؛ برای آسیب منیسک خارجی به ترتیب ۸۵، ۷۰/۸ و ۸۸/۵ درصد؛ در مورد آسیب رباط متقاطع جلویی به‌ترتیب ۹۵/۸، ۹۸/۶ و ۹۱/۷ درصد؛ و در مورد رباط متقاطع پشتی در هر سه مورد ۱۰۰ درصد بود. دقت، حساسیت و ویژگی ام‌آرآی برای آسیب منیسک داخلی به‌ترتیب ۷۷/۵، ۸۴/۲ و ۷۱/۴ درصد؛ آسیب منیسک خارجی به ترتیب ۸۵/۸، ۵۶/۵ و ۹۲/۸ درصد؛ آسیب رباط متقاطع جلویی به‌ترتیب ۹۲/۵، ۹۸/۶ و ۸۳/۳ درصد؛ و برای رباط متقاطع پشتی در هر سه مورد ۱۰۰ درصد بود. در مجموع، در آسیب‌های منفرد، دقت معاینه فیزیکی نسبتا بیشتر از آسیب‌های پیچیده و نتایج ام‌آرآی در این مورد برعکس بود.نتیجه گیری: براساس نتایج این مطالعه، هر دو روش معاینه فیزیکی و ام‌آرآی در تشخیص آسیب‌های زانو بسیار حساس و دقیق می‌باشد ولی معاینه فیزیکی کمی برتری دارد. ام‌آرآی بایستی در موارد مشکوک یا آسیب‌های پیچیده مورد استفاده قرار گیرد.}, keywords_fa = {آرتروسکوپی,تصویربرداری با تشدید مغناطیسی,منیسک تی‌بیا,رباط داخل زانو}, url = {https://www.ijos.ir/article_121171.html}, eprint = {https://www.ijos.ir/article_121171_f31f4c1fe83b8b9c758912a23a3a6eaa.pdf} }
