We would like to report our results with the reconstructive shoulder surgery in a group of older children.Methods: Shoulder contracture from brachial plexus birth palsy was surgically released in 24 older children, with simultaneous transfer of the latissimus dorsi and teres major tendons to the posterolateral humerus or rotator cuff. The mean age at surgery was 8 years 11 months. The children were evaluated at a mean follow-up time of 6 years 8 months by the Modified American Shoulder and Elbow Surgeons Form and Shoulder Pain and Disability Index standardized shoulder assessment forms, a questionnaire assessing activities of daily living, and parent and patient satisfaction scales.Results:  The fixed internal rotation deformity of 30º±12º improved to 72.5º±27º of external rotation. Improvements of 65º±26º and 47º±33º were seen in abduction and forward elevation, respectively, all without preoperative casting or manipulation. Increased external rotation had a more positive correlation with a higher functional score than abduction. Patients with lower preoperative functional scores had more rotational gain and higher functional scores after surgery. Those aged over 9 years had a functional gain similar to that of the younger children.Conclusion: Simultaneous release of contracted shoulder and transfer of latissimus dorsi and teres major tendons in brachial plexus birth paralysis is effective, and rewarding even in older children who have had late referral, and might have already developed glenoid dysplasia.}, keywords = {Paralysis, obstetric,Erb s palsy,Tendon Transfer}, title_fa = {آزادسازی شانه و انتقال عضله برای آسیب شبکه بازویی زمان تولد در کودکان سنین بالاتر}, abstract_fa = {پیش‌زمینه: تغییر شکل و خشکی شانه از یافته‌های شایع بعد از فلج زایمانی شبکه بازویی است که حتی در گروهی که بهبودی خوبی از لحاظ کارآیی عصبی داشته‌اند و یا جراحی میکروسکوپی ترمیم اعصاب در نوزادی برایشان انجام شده، دیده می‌شود. ما در این مقاله نتایج درمان آزادسازی و بازسازی شانه را در کودکانی با سن بالاتر گزارش می‌کنیم.مواد و روش‌ها: در ۲۴ بیماری که خشکی شانه ناشی از فلج زایمانی شبکه بازویی داشتند، عمل جراحی آزادسازی شانه و انتقال همزمان تاندون‌های «لاتیسیموس پشتی» و «ترس ماژور» به قسمت پشتی‌ـ‌خارجی هومروس و یا عضلات روتاتورکاف انجام گردید. متوسط سن زمان جراحی ۸ سال و ۱۱ ماه بود. میانگین زمان پیگیری ۶ سال و ۸ ماه بود. بیماران با پرسشنامه‌ها «جراحان شانه و آرنج امریکا» و «اندکس درد و ناتوانی در شانه» و پرسشنامه‌های مربوط به فعالیت‌هایی روزانه زندگی و امتیازدهی رضایت بیمار و خانواده وی، تحت بررسی و پیگیری قرار گرفتند.یافته‌ها: میزان انحراف ثابت شانه به‌صورت چرخش به داخل از ۱۲±۳۰ درجه به ۲۷±۷۲/۵ درجه چرخش به خارج بهبود یافت. ابداکسیون به میزان ۲۶±۶۵درجه و حرکت به جلو به میزان ۳۳±۴۷ درجه افزایش یافتند در حالی‌که هیچ‌گونه گچ‌گیری یا مانیپولاسیون قبل از عمل نداشتند. افزایش چرخش به خارج نسبت به افزایش ابداکسیون ارتباط بیشتری با امتیاز بالای کارآیی بیماران داشت. بیمارانی که قبل از جراحی، امتیاز کارآیی کمتری داشتند، توانستند چرخش خارجی بیشتر و امتیاز کارآیی بالاتری را با جراحی به‌دست آوردند. گروهی که سن ۹ سال و بالاتر داشتند از لحاظ به‌دست آوردن کارآیی، تفاوتی با بچه‌های کوچک‌تر نداشتند.نتیجه‌گیری: جراحی آزادسازی شانه همزمان با انتقال تاندون‌های «لاتیسیموس پشتی» و «ترس ماژور» در فلج زایمانی شبکه بازویی مؤثر است و حتی در بچه‌های بزرگ‌تر که مراجعه دیر و دیسپلازی گلنویید دارند، کمک‌ کننده است.}, keywords_fa = {فلج زایمانی,فلج ارب,انتقال تاندون}, url = {https://www.ijos.ir/article_121154.html}, eprint = {https://www.ijos.ir/article_121154_7b3005bb835eb425f95fd0827392a03a.pdf} }
