Tranexamic acid (TXA), an antifibrinolytic drug, is commonly used to control post-operative blood loss. However, the ideal route of administration is not yet quite clear. In this study, the different administration routes of TXA for blood loss control after TKA will be evaluated. Methods: In a clinical trial study, 200 patients who were scheduled for knee arthroplasty were randomly divided into 4 groups. In group 1, TXA (500 mg mixed in 100 cc of saline) was administered intravenously at the time of wound closure. In group 2, the knee joint cavity was irrigated and soaked for 5 minutes with 3 g of TXA in 100 cc of saline just before wound suturing. In group 3, immediately after wound closure, 1.5 g of TXA in 100 cc of saline was injected into the knee through the drain. Group 4 (control group) received no TXA , but the drain was clamped completely for an hour. The amount of blood loss and transfusion, and changes in hemoglobin levels were documented accordingly. Results: The mean post-operative blood loss in the groups 1 to 4 were respectively 476.8±114.8, 743.2±116.5, 173.9±60.5, and 860.5±152.2 ml (p < /i>}, keywords = {Tranexamic acid,Total knee replacement,Surgical blood loss,Intra-articular injection,Intravenous injections}, title_fa = {اثر سه روش استفاده از ترانکسامیک اسید در کاهش خونریزی پس از جراحی تعویض مفصل زانو}, abstract_fa = {۱پیش‌زمینه : یکی از مشکلات جراحی تعویض مفصل زانو خونریزی است. ترانکسامیک اسید داروی آنتی فیبیرینولیتیک است که برای کاهش خونریزی استفاده می‌شود و روش استفاده از آن مورد بحث می‌باشد. در این مطالعه تأثیر‌ روش‌های مختلف استفاده از این دارو بر میزان خونریزی پس از جراحی تعویض مفصل زانو بررسی شد. مواد و روش‌ها: در یک کارآزمایی بالینی، ۲۰۰ بیمار دچار استئوآرتریت زانو که کاندید تعویض مفصل زانو بودند، به‌طور تصادفی به ۴ گروه تقسیم شدند. به گروه 1 پس از اتمام جراحی، ۵۰۰ میلی‌گرم ترانکسامیک اسید در ۱۰۰ سی‌سی سالین داخل وریدی تزریق شد. در گروه 2 قبل از بخیه کردن، مفصل زانو با 3 گرم ترانکسامیک اسید در ۱۰۰ سی‌سی سالین شستشو داده شد. در گروه 3 بلافاصله بعد از بستن زخم، ۵/۱ گرم ترانکسامیک اسید در ۱۰۰ سی‌سی سالین از طریق درن تزریق شد. گروه ۴ تحت درمان با ترانکسامیک اسید قرار نگرفت و درن برای یک ساعت به‌طور کامل کلمپ شد. میزان خونریزی، میزان ترانسفیوژن و تغییرات هموگلوبین پس از عمل بررسی گردید. یافته‌ها: میانگین خونریزی پس از عمل در گروه‌های ۱، ۲، ۳ و ۴ به ترتیب ۸/۱۱۴ ± ۸/۴۷۶، ۵/۱۱۶ ± ۲/۷۴۳، ۵/۶۰ ± ۹/۱۷۳ و ۲/۱۵۲ ± ۵/۸۶۰ سی‌سی بود (۰/۰۰۱) . هموگلوبین بعد از عمل در گروه‌های ۱، ۲، ۳ و ۴ به ترتیب ۹/۰±۶/۲، ۱±۲/۴، ۱/۱±۹/۳ و ۱±۵/۴ میلی‌گرم بر دسی‌لیتر کاهش یافت(۰۰۱/۰). نتیجه‌گیری: در مقایسه با روش‌های تزریق داخل مفصلی ترانکسامیک اسید، استفاده داخل وریدی آن در جلوگیری از کاهش هموگلوبین پس از عمل موثرتر است.}, keywords_fa = {ترانکسامیک‌اسید,تعویض مفصل زانو,خونریزی پس از عمل,تزریق داخل مفصلی,تزریق داخل وریدی}, url = {https://www.ijos.ir/article_121082.html}, eprint = {https://www.ijos.ir/article_121082_e5fae0a3a6c34619ae2162cb591e2759.pdf} }
