There are several clinical tests with different diagnostic values for meniscus injuries. The aim of the present study was to evaluate the diagnostic value of modified "axial loading McMurray test". Methods: In a cross sectional diagnostic study, 146 patients (120 male, 26 female) with a mean age of 35 years who had clinical symptoms of knee meniscus injury and were candidates for arthroscopic evaluation entered the study in a teaching hospital in Guilan-Iran. "Axial loading Mc Murray – ALMM" test was performed in all the cases just before doing arthroscopic evaluation of the knees. The accuracy, sensitivity, specificity, positive and negative predictive values of the ALMM in relation to the later arthroscopic findings were analyzed. Results: From the 146 patients, 92 had evidence of meniscus tear on ALMM testing. The arthroscopy was, however, positive in 82 patients. In the remaining 54 cases with negative ALMM test, 38 patients had meniscus tear at arthroscopy. 56% of tears were seen in medial meniscus, 75% of which were in posterior horn. The ALMM test had 67.1% accuracy, 68.3% sensitivity, 61.5% specificity, 89.1% predictive positive value and 29.6% negative predictive value. Conclusion: Positive "Axial loading Mc Murray" for knee meniscus injury is expected to show meniscal tear in 89% of cases}, keywords = {Menisci, tibial,physical examination,knee}, title_fa = {بررسی ارزش تشخیصی تست «آگزیال لودینگ مک‌موری» در پارگی‌های منیسک زانو}, abstract_fa = {پیش‌زمینه: هدف از انجام معاینات و تست‌های بالینی، دستیابی به تشخیص صحیح است. برای صدمات منیسک‌های زانو، تست‌های بالینی متعدد با ارزش‌های تشخیصی متفاوت وجود دارند. هدف از این مطاالعه، بررسی ارزش تشخیصی تست تغییر یافته «آگزیال لوودینگ مک موری» بود. مواد و روش‌ها: در این مطالعه مقطعی از نوع تشخیصی، ۱۴۶بیمار (۱۲۰مرد، ۲۶زن) با میانگین سنی 35 سال که با نشانه‌های بالینی پارگی منیسک به کلینیک ارتوپدی شهر گیلان مراجعه کردند، پس از انجام معاینه بالینی و انجام ام‌آرآی با تشخیص احتمالی پارگی منیسک، کاندیدای آرتروسکوپی شدند. قبل از انجام آرتروسکوپی، در اتاق عمل تست «آگزیال لودینگ مک‌موری» انجام شد. سپس براساس یافته‌های زمان عمل آرتروسکوپی، دقت، حساسیت، ویژگی، ارزش اخباری مثبت و ارزش اخباری منفی تست در تشخیص پارگی‌های منیسک، ارزیابی گردید. یافته‌ها: از ۱۴۶بیمار، ۹۲بیمار با تست «آگزیال لودینگ مک‌موری» تشخیص پارگی منیسک داشتند و در آرتروسکوپی ۸۲ بیمار نیز پارگی مشاهده شد. از ۵۴ بیمار که تست «آگزیال لودینگ مک‌موری» منفی داشتند، پارگی منیسک در ۳۸بیمار با آرتروسکوپی دیده شد. ۵۶% پارگی‌ها در منیسک داخلی و ۷۵% در شاخ عقبی مشاهده شدند. تست فوق دقت ۱/۶۷%، حساسیت ۳/۶۸، ویژگی ۵/۶۱، ارزش اخباری مثبت ۱/۸۹% و ارزش اخباری منفی ۶/۲۹% نشان داد. از نظر شکل، پارگی نوع طولی دسته سطلی، شایع‌ترین (۶۰% ) بود. نتیجه‌گیری: با توجه به ارزش اخباری مثبت تست «آگزیال لودینگ مک‌موری» برای صدمات منیسک، انتظار می‌رود این تست پارگی منیسک را در ۸۹% بیماران نشان دهد.}, keywords_fa = {منیسک تی‌بیا,معایته جسمی,زانو}, url = {https://www.ijos.ir/article_121022.html}, eprint = {https://www.ijos.ir/article_121022_19a85df75f9bdc9a2fd4edcd88af3282.pdf} }
