چکیده

پیش‌زمینه: دررفتگی جلویی مکرر شانه نیاز به درمان جراحی دارد و سابقه طولانی نتایج عمل باز با موفقیت بالا، در متون ارتوپدی موجود است. با توجه به اینکه گزارش‌هایی زیادی از جراحی آرتروسکوپی شانه در ایران وجود ندارد، هدف از این مقاله گزارش نتایج کوتاه‌مدت جراحی آرتروسکوپی ضایعه بانکارت در ایران می‌باشد.مواد و روش‌ها: در یک مطالعه گذشته‌نگر، ۴۰ بیمار (۳۹ مرد و ۱ زن) مبتلا به دررفتگی جلویی شانه طی دو سال در دو مرکز درمانی شهر تهران، تحت عمل جراحی ترمیم آرتروسکوپیک ضایعات بانکارت و جابه‌جایی کپسول مفصلی قرار گرفتند. میانگین سن بیماران ۲۵/۳ سال و میانگین زمان پیگیری ۱۵/۵ ماه (۳۰-۶ ماه) بود. نتایج عمل با درجه‌بندی شانه دانشگاه کالیفرنیا- لوس‌آنجلس (UCLA)، معیار «راو» (Rowe) و «مقیاس جامعه شانه و آرنج آمریکا» (ASES) ارزیابی گردید.یافته‌ها: تمام بیماران دچار دررفتگی جلویی تروماتیک شانه بودند. میانگین نمره «راو» ۹۵/۵ (۱۰۰-۷۷) بود. نتایج معیار «راو» در ۳۶بیمار خوب و عالی بود و بدون محدودیت حرکتی به فعالیت‌های قبلی خود بازگشتند، در ۳ بیمار متوسط و در یک بیمار  با عود دررفتگی ضعیف همراه بود. میانگین نمره UCLA معادل ۳۳/۳ (۳۵-۲۵) و نتایج در همه بیماران غیر از ۴ مورد خوب و عالی بود. در مقیاس ASES، سه بیمار امتیاز ۳-۲ داشته و بقیه علایم و شکایت خاصی نداشتند.نتیجه‌گیری: یافته‌های زودرس و میان‌مدت در بیماران این مطالعه بسیار جالب بود. علی‌رغم نیاز به نتایج درازمدت، تاثیر این روش در درمان دررفتگی مکرر جلویی شانه با مطالعه ما نیز تأیید ‌شد.

Arthroscopic Repair in Anterior Shoulder Dislocation: Short-Term Results

چکیده [English]

Background: Treatment of recurrent shoulder dislocation with open technique has a long successful history in orthopaedic surgery. Arthroscopic repair, however, has a much shorter history especially in Iran. This is a short-term follow-up study of such an operation in Iran.Methods: Forty patients (39 males, 1 female) with recurrent anterior shoulder dislocation who had undergone arthroscopic stabilization over a two-year period were evaluated in a retrospective study. The cases had a mean age of 25.3 years, and had 15.5 months (6-30 months) average follow-up. The assessment scores were “The University of California-Los Angeles” (UCLA), “Rowe” and “American Shoulder and Elbow Society Scores” (ASES).Results: Mean Rowe score was 95.5 (77-100) with excellent scores in 32 patients, good in 4, fair in 3 and poor in one. Mean UCLA score was 33.3 (25-35), with good and excellent results in all but 4 cases. ASES was 2-3 in 3 patients, and the rest being painless and without any symptoms. Thirty six patients returned to their previous levels of activity, and achieved good results the results were fair in 3, and poor in one patient who had recurrent instability.Conclusions: Short-term results of arthroscopic repair of anterior shoulder instability are encouraging.
