چکیده
پیشزمینه: هدف از این مطالعه، منطبقسازی خصوصیات چند سیستم ارزیابی استئوآرتریت هیپ و زانو است که براساس نیازهای بیماران ایرانی تغییراتی در آن داده شده است. سئوال این است که آیا با بومی کردن مقیاسهای شناخته شده، میتوان کارکرد و تغییرات علایم حاصل از درمان آرتروپلاستی برای استئوآرتریت هیپ و زانو را بهطور واضحتر و با حساسیت بیشتری نشان داد.مواد و روشها: سی بیمار دچار استئوآرتریت اولیه هیپ یا زانو که تحت عمل جراحی آرتروپلاستی کامل قرار گرفته بودند و شرایط لازم ورود به مطالعه را داشتند، تحت بررسی قرار گرفتند. همه بیماران در روز قبل از عمل، در فاصله زمانی ۶ هفته، ۳ ماه و ۶ ماه پس از عمل با استفاده از مقیاسهای «وُومک» (WOMAC)، «لکسن» (Lequesne)، «دیل» (Doyle)، مقیاس بومیسازی شده و ارزیابی کلی بیمار و مصاحبهگر از وضعیت بیماری بررسی شدند. همچنین فاکتورهای بالینی زمان طی کردن فاصله ۵۰ فوت و دامنه حرکت زانو در بیماران بررسی شد. نتایج با تستهای پارامتریک، غیرپارامتریک و ضریب پیرسون مقایسه شدند.یافتهها: مقیاس بومیسازی شده از متوسط ۱۱۸ (۹۹-۱۳۶) قبل از عمل به ۵۳/۴ (۴۱-۶۸) در هفته ششم بعد از عمل و ۳۱/۵ (۴۴-۲۳) در ماه ششم پس از عمل بهبود یافته بود و تفاوت از نظر آماری معنیدار بود. مقیاسهای «وُومک» و «لُکسن» به ترتیب از ۷۵/۶ (۹۲-۵۵) و۱۸/۶ (۲۴-۱۱) در قبل از عمل به ۱۸/۸ (۲۴-۱۷) و ۶/۶ (۱۱-۳) در ماه ششم پس از عمل کاهش یافته بودند تفاوت هر دو از نظر آماری معنیدار بود. مقیاس دویل نیز کاهش قابل ملاحظهای داشت. آزمونهای مرجع تخمین کلی بیمار و پرسشگر به ترتیب از میانگین ۳/۴ و ۳/۹ در قبل از عمل به ۱/۵ و ۱/۹ در هفته ششم پس از عمل و ۰/۷ و ۰/۸ در ماه ششم پس از عمل بهبود یافتند. حساسترین مقیاس پس از عمل، مقیاس بومیسازی شده و سپس «لُکسن» بودند.نتیجهگیری: مقیاس بومی شده برای اندازهگیری علایم استئوآرتریت هیپ و زانو نسبت به سایر مقیاسهای بینالمللی، از «حساسیت» بیشتری برخوردار است.

Validation of a Nativized Index for Osteoarthritis of the Hip and Knee

Background: Nativization is modification of the components of a system in accordance with the needs of a society without altering its original function. We hypothesized that an index that incorporates Iranian society native specifications would be more sensitive in measurement of hip and knee osteoarthritis severity in comparison to common international indices.Methods: Thirty patients with primary osteoarthritis of hip or knee that underwent total joint arthroplasty and fulfilled the criteria were enrolled. Patients were studied using WOMAC, Lequesne, Modified Doyle index, nativized WOMAC index and patient and interviewer global assessment preoperatively and also 6 weeks, 3 months and 6 months post-operatively. "50 feet walking time" and knee range of motion were also studied. The results were analyzed using parametric and nonparametric tests and Pearson coefficient of correlation.Results: The nativized index improved from mean 118 (99-136) pre-operatively to 53.4 (41-68) at 6 weeks post-operatively and 31.5 (23-44) at 6 months. WOMAC and Lequesne improved from mean pre-operative values of 75.6 (55-92) and 18.6 (11-24) to 18.8(17-24) and 6.6 (3-11) at 6 months. Doyle index also improved significantly. Interviewer and patient global assessment improved from mean pre-operative value of 3.4 and 3.9 respectively to 1.5 and 1.9 at 6 weeks and 0.7 and 0.8 at 6 months post-operatively respectively. The most sensitive indices were nativized WOMAC and Lequesne indices.Conclusions: Nativized WOMAC index in knee and hip osteoarthritis demonstrates the severity of symptoms with more sensitivity than other common international indices.
