چکیده

پیش‌زمینه: شکستگی‌های سوپراکندیل هومروس از آسیب‌های شایع اندام فوقانی در کودکان می‌باشد. در مواقع نیاز به جراحی باز در این شکستگی، رویکردهای مختلفی ممکن است بکار گرفته شوند. هدف از این تحقیق، مقایسه دو رویکرد جراحی در چنین موارد است. مواد و روش‌ها: در یک مطالعه گذشته‌نگر، از ۸۷ بیمار متوالی (case series) مراجعه کننده به یک مرکز درمانی مشهد با شکستگی سوپراکندیل نوع ۳ و در محدوده سنی از زمان تولد تا ۱۸سال، با انتخاب تصادفی، ۴۳بیمار با رویکرد خارجی (لاترال) و 44 بیمار با رویکرد پشتی (پوستریور) تحت عمل جراحی باز و تثبیت پایین قرار گرفتند. با یک پیگیری ۶ماهه دامنه حرکات آرنج، عوارض و زوایای «بومن» (Baumann)، «کارینگ» (carrying) و کپیتوهومرال و نحوه جوش‌خوردن در دو گروه بررسی شدند. یافته‌ها: رویکرد پشتی در زمان کوتاه‌تری قابل انجام بود. دامنه حرکات در رویکرد پشتی بعد از یک و بعد از شش ماه، کمتر از رویکرد خارجی بود. زاویه «بومن» و «کارینگ» در رویکرد پشتی بیشتر در محدوده طبیعی بود. عوارض ناهنجاری واروس و والگوس، سفتی مفصلی و عفونت پین در دو رویکرد تفاوت معنی‌داری نداشت. نتیجه‌گیری: دو رویکرد از نظر عوارض بعد از عمل تفاوتی با هم نداشتند. در رویکرد خارجی دامنه حرکت بیشتر حفظ شد و در رویکرد پشتی جااندازی آناتومیک‌تری بدست آمد.

Surgical Approaches in Type III Supracondylar Humerus Fracture in Children (A Comparison between Posterior and Lateral Approaches)

Background: Supracondylar humerus Fracture is among the common upper extremity fractures in children. In certain types of fractures of circumstances that open reduction becomes necessary the best surgical approach is debatable. We would like to report our prospective study between posterior and lateral surgical approaches in type III supracondylar fractures. Methods: In a case series study, 87 children from birth till 18 years old, with type III humerus fracture were randomly selected for open reduction and pinning 43 through lateral and 44 posterior approaches. With a 6-months follow-up, elbow range of motion, complications and radiographic parameters related to :::union::: type were evaluated. Results: Posterior approach was a faster surgery. Ranges of motion after 1 and after 6 months were more restricted in posterior approach. Baumann and carrying angles were closer to normal in a higher percentage in posterior approach. Varus and valgus mal:::union::: or joint stiffness rates or infection were similar in the two groups. Conclusion: The two approaches were similar for postoperative complications. Better range of motion was achieved in the lateral approach, and a more anatomical :::union::: in the posterior approach.
