Two pining methods are used for fixation of extraarticular proximal phalanx transverse fractures: transarticular parallel pins passing through (MCP) joint and extraarticular cross pining of fracture with out going through MCP joint. The purpose of this study was to comparie these two fixation methods. Methods: This clinical trial study was done on proximal phalanx transvers fracture using two methods in a training center in Kerman-Iran. In first method, two parallel pins were used from metacarpal head passing through MCP joint into proximall phalanx. In second method two cross pins were used from proximall phalangeal condyles across the fracture. The active range of motion for MCP, PIP and DIP joints, the duration of resuming patients their daily activities, and post operation complications were evaluated. Results: The mean active range of motion for MCP, PIP and DIP joints, 3 and 6 months after surgery showed no significant difference between the two methods of surgery. There was no significant difference between the two methods in age, gender and the duration of returining to work. Conclusions: There was no difference in the results for using cross pining or parallel transarticular pin fixation for proximal phalanx fractures. Since intramedullary method is easier with less use of C-ARM, less damage to soft tissue damage and less distraction of fracture, using transarticular intramedullary parallel pins fixation for proximal phalanx fracture is suggested.}, keywords = {Finger injuries,Finger phalanges,Fracture fixation,Internal,Bone pins}, title_fa = {مقایسه دو روش پین‎گذاری از راه پوست در درمان شکستگی‎های بند ابتدایی انگشتان}, abstract_fa = {پیش‎زمینه: شکستگی‎های انگشتان دست از شکستگی‎های شایع اندام فوقانی است. برای درمان شکستگی‎های خارج مفصلی بندهای انگشتان پایین می‎توان از داخل مفصل متاکارپوفالانجیال عبور کرد یا به‎طور ضربدری از خارج مفصل شکستگی را ثابت نمود. هدف از این بررسی، مقایسه بین دو روش بود. مواد و روش‎ها: این مطالعه  کارآزمایی بالینی بر روی نمونه‎های انسانی با شکستگی عرضی پروگزیمال فالنکس انگشتان در یک مرکز درمانی کرمان به دو روش انجام شد. در روش اول دو پین به‎صورت موازی داخل مفصلی از سر متاکارپ در تنه پروگزیمال فالنکس؛ و در روش دوم دو پین متقاطع به صورت خارج مفصلی از کندیل پروگزیمال فالنکس گذاشته شد. محدوده حرکتی فعال مفاصل  MCP < /span>، PIP < /span> و DIP < /span> ، مدت زمان لازم برای بازگشت بیمار به فعالیت‎های روزانه  و همچنین عوارض بعد از عمل مانند عفونت محل زخم جراحی بررسی شدند. یافته‎ها: میانگین محدوده حرکتی در مفاصل MCP < /span>، PIP < /span> و DIP < /span> در سه ماه و شش ماه پس از عمل جراحی نشان داد بین دو روش جراحی از نظر میزان محدوده حرکتی مفاصل پس از جراحی تفاوت معنی‌داری وجود نداشت. همچنین از نظر متغیرهای سن، جنس و مدت زمان برگشت به کار نیز بین  دو روش تفاوت معناداری مشاهده نشد (p < /em><۰.۰۵) نتیجه‌گیری: نتایج استفاده از پین کراس و اینترامدولاری، در شکستگی بند ابتدایی انگشتان یکسان بود با توجه به راحت تر بودن استفاده از دو پین موازی، استفاده کمتر از اشعه فلوروسکوپی سی آرم، آسیب کمتر بافت نرم و دیستراکشن کمتر شکستگی، استفاده از پین‎گذاری اینترامدولاری ترانس آرتیکولار توصیه می‌شود.}, keywords_fa = {آسیب انگشت,فالانج انگشت,تثبیت شکستگی داخلی,پین استخوان}, url = {https://www.ijos.ir/article_121313.html}, eprint = {https://www.ijos.ir/article_121313_036b87420fd20f20b40e0889b87cbc0e.pdf} }
