Measuring the distal femoral rotation is one of the most important problems in such operations. In the conventional 2D method, measuring the angle is based on CT scan images but these methods usually contain errors. In this study, the three-dimensional measurement of the aforementioned angle was investigated. Methods: In this research, using CT scans and 3D modeling, 3D lower extremity models of 40 patients were extracted. The rotation of distal femur was measured for plane perpendicular to the anatomical and the plane of mechanical axis of femur. Four axes were drawn on these planes using anatomical landmarks: posterior condylar line (PCL), anatomical and surgical transepicondylar line (ATEA+STEA), and the Whiteside line (WL).   Results: The mean difference of these measurements on the plane perpendicular to the mechanical axis of the femur, between PCL and WL,  STEA,  ATEA was 3.41, -1.31, 5.53; and angles on the plane perpendicular to the anatomical axis of femur were -0.74, -1.26, and 5.67, respectively. In addition, Bland-Altman diagram was plotted between every two measurements and no relationship found, except for STEA and ATEA. Conclusions: The measurements between PCL, ATE, and STEA are not affected by the plane on which these measurements are carried out on, except for the 4 degrees difference present in WL axis. With a greater sample size and proper grouping, some relationship might be found between the aforementioned axes.}, keywords = {Femur,rotation,Spiral tomography}, title_fa = {بررسی زاویه چرخش دیستال فمور با سی‌تی اسکن سه بعدی}, abstract_fa = {پیش‌زمینه: تعویض مفصل زانو بک درمان استاندارد برای زانوهای مبتلا به ساییدگی است و اندازه‎گیری زاویه چرخش ناحیه دیستال فمور یکی از مهم‎ترین مباحث می‌باشد. در روش‎های متداول، این زاویه به‎صورت دوبعدی و با تصاویر سی‌تی‌ اسکن اندازه‎گیری می‌شود که البته با خطا همراه است. هدف از این تحقیق، بررسی نحوه اندازه‎گیری این زاویه به‎صورت سه‎بعدی بود. مواد و روش‎ها: با استفاده از تصاویر سی‌تی‌اسکن، مدل­ سه بعدی فمور ۴۰ بیمار با نرم‎افزارهای مهندسی استخراج گردید. با قرار دادن نشانه‎ها، زاویه چرخش ناحیه انتهایی، با استفاده از محورهای برآمدگی پشتی، آناتومیک بین اپی‌کوندیل‌ها، جراحی بین اپی‌کوندیل‌ها و خط وایت ساید در دو نمای عمود بر محور آناتومی و مکانیکی محاسبه شد. یافته‎ها: در حالت نمای مشاهده عمود بر محور مکانیکی، میانگین زاویه بین خط برآمدگی پشتی با خط وایت ساید،  محور جراحی بین اپی کوندیل‎ها و محور آناتومیک بین اپی‌کوندیل‎ها به ترتیب برابر با ۳/۴۱ ، ۱/۳۱- و ۵/۵۳ ؛ و در حالت نمای مشاهده عمود بر محور آناتومی، به ترتیب ۰/۷۴- ، ۱/۲۶- و ۵/۶۷ درجه بود. نمودار بلند ـ آلتمن نشان داد، جز در محورهای بین اپی‌کوندیل‌ها و جراحی بین اپی‌کوندیل‌ها، رابطه‌ای بین سایر زوایا وجود ندارد. نتیجه‎گیری: محورهای برآمدگی پشتی، آناتومیک بین اپی‌کوندیل‌ها، جراحی بین اپی‌کوندیل‌ها، تحت تأثیر نمای اندازه‎گیری نمی‎باشند، اما اختلاف حدود ۴ درجه‎ نشان می‌دهد اندازه‎گیری‎هایی که بر مبنای محور خط وایت ساید انجام می‎گیرد، تحت تأثیر نحوه قرارگیری اندام می‎باشد. عدم ارتباط این خطوط نشان می‎دهد که باید محورهای دیگر و بیشتری برای بهبود عمل جراحی استفاده شود.}, keywords_fa = {فمور,چرخش,توموگرافی کامپیوتری}, url = {https://www.ijos.ir/article_121301.html}, eprint = {https://www.ijos.ir/article_121301_eeffbaa84b67be42e2e198ce32c9bc7d.pdf} }
