It is, therefore, associated with high chance of knee injuries. This articles looks at the prevalence of intra-articular knee injury in “floating knee” when the fractures are extra-articular, both in tibia and femur.Methods: During a 3.5 year period of time (March 2002-Oct 2005), 61, out of total of 76  cases of “floating knees” that had extra-articular femur and tibia fractures, had no history of prior knee problems, and the fractures of both segments had been  internally fixed were studied. Each knee was examined for any possible ligamentous injury, following rigid fixation of the fractures, under the same anesthesia. The findings were documented and the patients were later followed for any new findings or complaints up to nine months after the incident.Results: The mean age in these 61 cases (57 males, 4 females) was 29.3 (18-46). Lachman and Pivot tests were positive in 5 and Posterior Drawer test in 3 cases. Valgus instability in 9 and varus instability in 6 other cases were detected. After the 9-months follow-up, knee pain was the chief complaint in 19 patients, while the MRI study which was done in 16 cases was positive for meniscal damage in 13 patients.Conclusions: Concomitant knee insults are fairly common in “floating knee” injuries. A thorough knee examination, following fixation of "floating knee” fractures, while the patient is still under anesthesia is essential. Further evaluation during the course of fracture treatment for any knee complaint is also recommended.}, keywords = {Knee joint,Ligaments,articular,Trauma}, title_fa = {آسیب‌های زانو در بیماران زانوی شناور}, abstract_fa = {پیش‌زمینه: شکستگی یک‌طرفه استخوان‌های ساق و ران یا «زانوی شناور» (floating knee) عموما به دلیل ترومای با انرژی بالا اتفاق می‌افتد که به نظر می‌رسد با توجه به مکانیسم و شدت ضایعه احتمال آسیب همزمان در مفصل زانو وجود دارد. در این مطالعه آسیب‌های زانو در بیماران با شکستگی خارج مفصلی مورد ارزیابی قرار گرفت.مواد و روش‌ها: در این مطالعه از ۷۶ بیماری که در فاصله ۳/۵ سال از فروردین ۱۳۸۱تا مهر ۱۳۸۴با تشخیص زانوی شناور در یک مرکز درمانی تهران تحت درمان قرار گرفته‌ بودند، ۶۱ بیمار وارد مطالعه شدند. ملاک‌های ورود عبارت بودند از: ۱) شکستگی خارج مفصلی فمور و هر دو استخوان تی‌بیا و فیبولا در یک اندام تحتانی؛ ۲) درمان شکستگی‌های بیمار با فیکس شدن داخلی با میله یا پلاک؛ ۳) نداشتن هیچ‌گونه سابقه‌ای از مشکلات زانو. پس از ثابت نمودن شکستگی‌ها، در همه بیماران معاینه کامل زانو زیر بیهوشی توسط متخصص ارتوپدی و دستیار ارشد به‌عمل آمد و نتایج ثبت گردید.یافته‌ها: میانگین سن بیماران ۲۹/۳ سال (۴۶-۱۸) ۵۷ بیمار مرد و ۴ بیمار زن بودند. از ۶۱بیمار، در ۵بیمار  تست «پیووت» (Pivot) و «لاکمن»(Lachman) مثبت و در ۳ بیمار تست «کشویی پشتی» (Posterior Drawer) مثبت بود. همچنین ۹ بیمار بی‌ثباتی در والگوس و ۶ بیمار در واروس داشتند. پس از ۹ ماه پیگیری ۱۹ بیمار از درد زانو شکایت داشتند که در ام‌آر‌آی به عمل آمده از ۱۶ بیمار، ۱۳ بیمار پارگی منیسک داشتند.نتیجه‌گیری: در تعداد قابل ملاحظه‌ای از بیماران مبتلا به زانوی شناور به طور همزمان آسیب‌های متعددی در زانو وجود دارند که با معاینه دقیق بیماران زیر بیهوشی، پس از ثابت نمودن شکستگی‌ها و بررسی‌های بیشتر می‌توان این ضایعات را تشخیص و درمان نمود.}, keywords_fa = {مفصل زانو,لیگامان‌های مفصلی,ضربه}, url = {https://www.ijos.ir/article_121248.html}, eprint = {https://www.ijos.ir/article_121248_bfdce552c83235ca95e532058b1cd71c.pdf} }
