In order to decrease the spasticity, oral medications, direct intramascular injections of alcohol, phenol or Botulinum toxin, or surgery are used. This study aimed at comparing the efficacy of 45% ethanol injection with that of Botulinum toxin injection in the spastic calf muscles of diplegic cerebral palsy children.Methods: Seventy five children (34 girls, 41 boys) with diplegia were divided into three groups: Group 1 received one injection of 45% alcohol followed by 4 weeks of cast immobilization, and then physiotherapy group 2 received Botulinum toxin followed by physiotherapy and group 3 had only physiotherapy with no injection or immobilization. The walking pattern of all three groups were evaluated clinically in one year in terms of independency from walking support, heel-toe gait pattern, neutral position of hind foot at heel strike, and ankle position in stance phase.Results: At one year, support independency was observed in 60% of group 1, 25% of group 2 and 40% of group 3 treated cases. The results in heel-toe gait were 100%, 90% and 50% respectively. Neutral heel placement was observed in 60%, 25% and 12.5%. Neutral ankle position was seen in 100%, 100% and 10% respectively.Conclusions: 45% alcohol injection into calf muscles of spastic diplegic cerebral palsy children gives as good or better result than Botulinum toxin injection. Because of its lower cost and easier accessibility, it is preferred over Botulinum toxin.}, keywords = {Botulinum toxin type A,Cerebral palsy,Muscle spasticity}, title_fa = {مقایسه اثر تزریق الکل با توکسین بوتولینوم در فلج مغزی}, abstract_fa = {پیش‌زمینه: گرفتگی عضلانی یکی از شایع‌ترین علایم فلج مغزی است. غیر از درمان‌های جراحی و داروهای خوراکی که برای از بین بردن یا کاهش گرفتگی به‌کار می‌رود، مطالعات متعددی درمورد تأثیر تزریق مستقیم موادی مثل الکل (اتانول ۴۵درصد یا فنل ۵ تا ۷ درصد) و توکسین بوتولینوم، در عضلات گرفته انجام شده است. در این مطالعه اثربخشی اتانول ۴۵ درصد با توکسین بوتولینوم، در درمان اکواینوس ثانویه به گرفتگی عضلات در فلج مغزی دی پلژیک مقایسه شد. مواد و روش‌ها: در یک کارآزمایی بالینی ۷۵ کودک (۳۴ دختر و ۴۱ پسر) مبتلا به فلج مغری دی‌پلژیک جهت درمان به سه گروه تقسیم شدند؛ درمان در گروه اول (۱۵ بیمار)،‌ به صورت تزریق الکل ۴۵ درصد، ۴ هفته گچ‌گیری و به‌دنبال آن فیزیوتراپی و کار درمانی؛ در گروه دوم (۲۰ بیمار) به صورت تزریق توکسین بوتولینوم و سپس فیزیوتراپی و کاردرمانی و در گروه سوم (۴۰ بیمار) به صورت فیزیوتراپی و کاردرمانی انجام شد. میانگین سنی کودکان ۴/۲ سال (۶-۲سال) بود.یافته‌ها: در یک مطالعه یک‌ساله، عدم نیاز به ساپورت در ۶۰% گروه اول، ۲۵% گروه دوم و ۴۰% گروه سوم مشاهده شد. نتایج در الگوی راه‌رفتن صحیح پاشنه ـ انگشت در سه گروه به ترتیب ۱۰۰% ، ۹۰% و ۵۰% ، و در وضعیت خنثی در پاشنه پا به ترتیب ۶۰% ، ۲۵% و ۱۲/۵% بود. وضعیت خنثی پا در مرحله سکون راه رفتن در ۱۰۰% گروه اول و دوم و ۱۰% گروه سوم مشاهده شد.نتیجه‌گیری: با توجه به دسترسی آسان‌تر و ارزان‌تر بودن، تزریق الکل ۴۵ درصد همراه با یک دوره بی‌حرکتی (گچ‌گیری) در مقایسه با توکسین بوتولینوم، می‌تواند روش بهتری برای درمان گرفتگی عضلانی در فلج مغزی باشد.}, keywords_fa = {توکسین بوتولینوم نوع A,فلج مغزی,گرفتگی عضلانی}, url = {https://www.ijos.ir/article_121201.html}, eprint = {https://www.ijos.ir/article_121201_4bf666c4f3298bc82bac3595be5171d1.pdf} }
