The aim of this study was to evaluate the effect of adding topical gentamicin to the surgical wound after open reduction and internal fixation of a long bone fracture. Methods: In a clinical trial, 120 patients who were candidates for open reduction and internal fixation of a long bone fracture in a training hospital in Bandar Abbass, Iran were divided into two equal groups (60 patients each). Routine prophylactic standard systemic antibiotic regimen was used in both groups. In the "case group", gentamicin was used topically in operation site in addition to the systemic regimen. All the patients were followed at least for 6 months and observed for superficial and or deep infection signs and symptoms. Results: In the control group, 12 infections were observed, 7 of which were superficial and cleared with antibiotic and wound care. In the study group only 3 superficial and no deep infections was found. Statistical analysis revealed that both superficial and deep infection rates were significantly lower in the study group (p < /em>=.01). Conclusions: Prophylactic topical use of gentamicin in addition to the systemic regimen can be effective in reducing the rate of infection after open reduction and internal fixation of long bone fractures. }, keywords = {Anti bacterial agents,Infection,Gentamicin,Fracture, open}, title_fa = {تأثیر جنتامایسین موضعی پس از عمل جراحی جااندازی باز و تثبیت شکستگی استخوان‌های بلند}, abstract_fa = {پیش‌زمینه: عفونت پس از اعمال جراحی ارتوپدی به‌خاطر جریان خون ضعیف استخوان‌های بلند و اکثراً استفاده از وسایل فلزی خطرناک‌ است. هدف از این مطالعه بررسی تاثیر اضافه نمودن جنتامایسین به‌صورت موضعی به زخم بیمار پس از عمل جااندازی باز و تثبیت استخوان بلند شکسته بود. مواد و روش‌ها: در یک کارآزمایی بالینی، ۱۲۰بیمار که به علت شکستگی یک استخوان بلند کاندید عمل جراحی جااندازی باز و تثبیت داخلی بودند به دوگروه همسان و ۶۰ نفره شاهد و مطالعه تقسیم شدند. در گروه شاهد تنها از رژیم سیستمیک استاندارد به عنوان درمان پیشگیرانه استفاده شد، در حالی که در گروه مطالعه علاوه بر رژیم سیستمیک از جنتامایسین موضعی نیز استفاده گردید. تمامی بیماران در یک دوره حداقل 6 ماهه از نظر علایم عفونت سطحی وعمقی پیگیری شدند. یافته‌ها: در گروه شاهد ۱۲ مورد عفونت مشاهده گردید که ۷ مورد سطحی بودند و با درمان موضعی و آنتی‌بیوتیک بهبود یافتند. در گروهی که آنتی‌بیوتیک موضعی گرفتند، ۳مورد عفونت سطحی مشاهده شد و هیچ مورد عفونت عمقی مشاهده نگردید. مقایسه نتایج نشان داد که عفونت‌های سطحی و عمقی با اضافه کردن جنتامایسین موضعی به رژیم آنتی‌بیوتیک سیستمیک به طور معنی‌داری کاهش یافت (۰.۰۱=p) نتیجه‌گیری: استفاده پیشگیرانه از جنتامایسین به‌صورت موضعی در کنار رژیم استاندارد آنتی‌بیوتیک سیستمیک می‌تواند عفونت پس از عمل جراحی جااندازی باز و تثبیت داخلی استخوان‌های بلند را کاهش دهد.}, keywords_fa = {مواد ضدعفونی,عفونت,جنتامایسین,شکستگی باز}, url = {https://www.ijos.ir/article_121079.html}, eprint = {https://www.ijos.ir/article_121079_db7223809a0eac5c0bddaa959ee6875b.pdf} }
