The aim of this study was to determine the safest corridor for pinning of fifth carpometacarpal joint to prevent the iatrogenic injury to the ulnar nerve and tendons .  Methods: In the first phase of study, three fresh cadaver samples were dissected and the safest directions of kirschner wire insertion in coronal and sagittal planes were determined to the base of the fifth metacarpal. The second phase evaluated the accuracy of data obtained in previous phase. Therefore k-wires were inserted in combination of maximum angles in different planes determined previously. After taking X-rays, the ulnar nerve branches and tendons were explored to detect any possible damage. In the third phase, four fresh carpometacarpal joints were fixed with directions outside the range of defined angl es.  Results: The safe direction determined in the first phase was 20° to 30° coronal plane angle in regard to body of the fifth metacarpal bone and between 10° volar to dorsal to 20° dorsal to volar angle in sagittal plane. Insertion of k-wires in the second phase could fix the fifth carpometacarpal joint firmly. All inserted k-wires outside the defined range resulted in injuries to nerve or tendons or infirm fixation  Conclusions: The safest corridor for pinning the unstable fifth carpometacarpal injuries is two centimeters distal to the joint with 20° to 30° in coronal plane and from 10° volar to dorsal to 20° dorsal to volar direction in sagittal plane.}, keywords = {Carpometacarpal joints,Skeletal fixation,Kirschner wires}, title_fa = {پین‌گذاری پوستی مفصل کارپومتاکارپال پنجم}, abstract_fa = {  پیش‌زمینه: درمان صدمات بی‌ثبات پنجمین مفصل کارپومتاکارپال دست، جااندازی بسته و پین‌گذاری می‌باشد. هدف این مطالعه، تعیین بی‌خطرین مسیر پین‌گذاری این مفصل با شیوه‌ای است که از صدمات احتمالی به عصب اولنار و تاندون‌های دست جلوگیری شود.   مواد و روش‌ها: در مرحله اول، مچ‌دست و دست سه جسد تازه تشریح شدند و سپس بی‌خطرترین مسیر پین‌گذاری در سطوح کرونال و ساژیتال نسبت به متاکارپال پنجم دست تعیین گردید. در مرحله دوم، پین‌گذاری طوری انجام شد که زوایای ورودی در هر نمونه، ترکیبی از ماکزیمم زوایای بدست آمده در مرحله قبل باشد. بعد از پرتونگاری، دست و مچ تشریح شدند. در مرحله سوم، پین‌گذاری در زاویه‌های خارج از محدوده‌های مرحله اول در دست چهار جسد انجام شد. بعد از پرتونگاری، دست‌ها کالبدشکافی شدند.   یافته‌ها: بی‌خطرترین مسیر تعیین شده در مرحله اول زاویه ۲۰ تا ۳۰درجه سطح کرونال نسبت به متاکارپال پنجم دست و در سطح ساژیتال از ۱۰ درجه ولار به سمت دورسال تا ۲۰ درجه دورسال به سمت ولار بود. در مرحله دوم پین‌گذاری در تمام نمونه‌ها با عبور پین از وسط مفصل باعث تثبت محکم مفصل شد. تمام پین‌های گذاشته شده در مرحله سوم باعث صدمه به عصب اولنار یا تاندون‌های دست شدند و با با عبور از کناره‌های مفصل، نتوانستند تثبیت محکم ایجاد کنند.   نتیجه‌گیری: بی‌خطرترین راهرو برای پین‌گذاری در صدمات بی‌ثبات مفصل پنجم کارپومتاکارپال دست با محل ورود ۲ سانتی‌متری دیستال از سطح مفصل، ۲۰ تا ۳۰ درجه در سطح کرونال و از ۱۰ درجه ولار به دورسال تا ۲۰ درجه دورسال به ولار در سطح ساژیتال می‌باشد.  }, keywords_fa = {مفصل کارپومتاکارپال,تثبیت اسکلتی,سیم کرشنر}, url = {https://www.ijos.ir/article_121065.html}, eprint = {https://www.ijos.ir/article_121065_f894864397e92d338d109ea1b00c348b.pdf} }
